A taxa de perfeição, e de infalibilidade, com essa magnitude de porcentagem, fugiu da definição conceitual de ppm, ou defeito zero, onde o evento controlado deveria tender a zero. Percebe-se que o pequeno índice de rejeição de 1% esconde uma catástrofe, quando relacionado à escala do milhão ou bilhão. Uma fábrica que produz ventiladores com uma taxa de rejeição de 2%, por exemplo, para o conceito de defeito zero ou ppm, terá que suportar cerca de 20.000 ventiladores em 1 milhão de ventiladores produzidos. Se cada ventilador custar US$ 25,00 tem-se um prejuízo de US$ 500 mil – perda por má qualidade. Cerca de 20.000 consumidores (donos de cada um dos ventiladores comprados com defeitos), comentam com pelo menos 20.000 parceiros ou cônjuges. E que podem comentar com pelo menos 40.000 filhos ou parentes no lar. Esses últimos comentam junto com os pais com cerca de 120.000 amigos. Os amigos comentam com pelo menos 240.000 outros amigos, cônjuges ou parceiros. Assim, a progressão da má qualidade, até aqui, atingiu o total de 440.000 pessoas extremamente próximas, quase todos os dias. Como resultado o incêndio da marca aconteceu na relação de 1 dono do ventilador defeituoso para 22 pessoas próximas – perda por má qualidade.
