This book assesses the correlation between cardiac ischemic score and coronary collaterals and to provide an alternative treatment option, in patients with ischemic heart disease, who are not suitable for conventional revascularization therapies. Coronary collateral circulation maintains myocardial perfusion in coronary atherosclerotic disease. The indicators of cardiac ischemia, like (Angina pectoris on exertion, during emotion, previous myocardial infarction, prior coronary intervention), are associated with presence of coronary collateral circulation in different studies abroad. The study correlating the coronary collaterals and the indicators of cardiac ischemia was not performed in Bangladeshi population and hence this study was carried out here. This prospective observational cross sectional study was carried out in department of Cardiology, BSMMU from January 2007 to December 2008. A total of 128 patients with angina pectoris and or MI and or previous coronary intervention undergoing CAG and PCI were included in the study.

The sober explanation for this book is a call by the Springer-Verlag, London, to edit a publication on ‘The functional relevance of the collateral circulation’ of the heart. Alternatively, it could be ‘sold’ as the result of my intention to reduce entropy of 18 years of scientific work on the topic of the coronary circulation, which was itself meant to diminish the amount of ‘useless’ energy. Such a process of reducing disarray in a system with the aim of grasping it better is related to simplification, which carries the risk of introducing error. This can be exemplified by the historic view of angina pectoris, which used to be simplified as being always fatal, thus obscuring for nearly two centuries the view of a ‘self-healing’ mechanism such as the collateral circulation of the heart. It would be na? ̈ve, to assume the present work to be free of erroneous oversimplification, because the very nature of scientific work is related to generating (simple) hypotheses with their subsequent falsification. In that context and bluntly, my primary interest in the field of the collateral circulation was not initiated with a vision of eradicating the consequences of coronary artery disease (CAD) by promoting the growth of natural bypasses. The time for such sizeable ideas had passed in the 1970s with the start of the work by Wolfgang Schaper.

Collateral blood vessels develop by growth of pre or newly formed structures in almost all vascular provinces as a consequence of progressing stenosis of the main artery. These alternative routes of blood supply are potentially able to alter the course of vascular disease. Collateral development is a time consuming process, and arterial stenosis and occlusion often progress faster than growth of the alternative routes. The authors' ultimate goal is to provide a better understanding of collateral growth in order to pave the way for improving the conditions for these potentially selfhealing processes. These were programmed by nature but have not been perfected, probably because defenses against arterial disease had not been put under the pressure of natural selection.

How can you adequately honor a man of deeds with mere words? Dr. Donald Eaton Gregg is recognized throughout the scientific world for his distinguished contributions to our knowledge of the coronary circulation, his advances in experimental instrumentation and design, his scientific integrity and his lasting influence on other investigators. His research associates over the years are now liberally scattered over the entire world. Their accomplishments and activities reflect the quality of their training in his Department of Cardiorespiratory Diseases at the Walter Reed Army Institute of Research. The positions of prestige occupied by these men attest to Dr. Gregg's ability to select and inspire men of talent, yet in his usual modest way he insists that he was only fortunate to be in the right place at the right time.
