The model is based on an incumbent serving a franchise area that can be divided into an urban and a rural sub-market. Price is determined by three factors: 1) competition in a (sub-)market; 2) the marginal cost in a (sub-)market; and 3) elasticity of demand in a (sub-)market. Competition clearly lowers price in the competitive areas. If the incumbent charges a uniform price over the entire franchise area then competitive effects are felt in the entire franchise area. In the case where entry induces non-uniform pricing by the incumbent, the differences in costs and elasticity from their franchise wide values can affect prices. With non-uniform pricing, lack of competition, higher marginal costs and more inelastic demand in a sub-market each imply higher prices in that sub-market.

Cream skimming is an illegal behaviour that consists in choosing to treat patients according to their ability to recover. It arises from the use of prospective payment schemes in an asymmetry of information framework. In this context in fact the provider can observe some relevant information (freely or at a cost) before making his effort which will then be used to its own advantage. The paper studies the scope for these types of behaviour in a mixed market for hospital care where the hospitals do not share the same objectives. We show that in this context cream skimming is made possible by the presence of two important elements: the public hospital prefers to treat high severity patients and the regulator is unable to enforce hard budget constraint rules. The paper adds an important dimension to the study of cream skimming as proposed by the traditional literature where asymmetry of information alone is considered the cause of this market failure. In our context in fact cream skimming mainly arises from a regulatory failure.

The use of prospective payment system in health care has mixed evidences as concerns its effectiveness. On the one side it avoid the problem of misreporting cost, but has the downside effect that, if hospitals might choose which patients to treat, competition might become unfair and the whole system might collapse. The aim of the paper is to review cream skimming in private and public health care markets in different institutional settings. It is shown that the environment in which cream skimming takes place and the objectives of the parties involved should be carefully examined in order to study an effective incentive scheme to correct for this failure.

Abstract: We propose an equilibrium occupational choice model, where agents can choose to work in the real sector (become entrepreneurs) or to become informed dealers in financial markets. Agents incur costs to become informed dealers and develop skills for valuing assets up for trade. The financial sector comprises a transparent competitive exchange, where uninformed agents trade and an opaque over-the-counter (OTC) market, where informed dealers offer attractive terms for the most valuable assets entrepreneurs put up for sale. Thanks to their information advantage and valuation skills, dealers are able to provide incentives to entrepreneurs to originate good assets. However, the opaqueness of the OTC market allows dealers to extract informational rents from entrepreneurs. Trade in the OTC market imposes a negative externality on the organized exchange, where only the less valuable assets end up for trade. We show that in equilibrium the dealers' informational rents in the OTC market are too large and attract too much talent to the financial industry
