Since coronary angioplasty was first practiced by Andreas Gruntzig in 1977, it has rapidly developed into a technique widely used on patients with chronic and acute coronary heart disease. The meeting described in this book was held under the auspices of the working group of our national society, chaired by Prof. Kaltenbach, Frankfurt, and by Prof. Meyer, Mainz, in cooperation with Stanford University. It is an attempt to present various cardiologists' appraisals of the current position ofPTCA in clinical medicine. PTCA is far from easy, as its mechanism is critically balanced between success and fail ure. Therefore the experience of more than one hundred cases is generally regarded as ne cessary for cutting down complications and achieving a high rate of sli'ccess. Furthermore, success and complications are a result not only of technical expertise, but also of patient selection. The emphasis of the workshop was on discussion of some unsolved problems and open questions such as: - What are the reliable indication guide-lines? - What are the established guide-lines for deciding, once a complication occurs, between operative and non-operative treatment? - What are the indications and limitations for the combined use of thrombolysis with PTCA in acute myocardial infarction? - When and at what intervals should PTCA be applied after successful lysis? - Who are the candidates for dilatation of main stem stenosis? - When should one attempt to open occluded arteries and which technique shows the most promise?

"Silent Ischemia, Current Concepts and Management" contains the proceedings of a conference held in Rottach-Egern, West Germany, March 5 to 7, 1987. When discussing silent myocardial ischemia, the first question is: "why is it silent?". To approach this question, a deeper look has to be taken into the pathophysiology of cardiac pain and the excitatory and inhibitory mechanisms involved. It has to be borne in mind that - in contradiction to what the poets have told us so beautifully for many centuries - the heart is a visceral and not a sensitive organ. If asymptomatic ischemia did not carry prognostic significance comparable to the symp tomatic manifestations of ischemia, then the problem of silent ischemia would be very academic and without consequences for treatment. Therefore studies on prognosis of silent ischemia are of great importance, as their results should indicate how aggressively patients are to be managed.

Since the introduction of balloon angioplasty for the relief of coronary artery stenoses and of anginal symptoms in patients with coronary artery disease by Andreas Griintzig in 1977, the field of interventional technology and treatment strategies has grown enormously. For the reduction of hemodynamically significant coronary artery stenoses balloon angioplasty is the standard and reference method with a high primary success and low complication rate. Because of the relatively high recurrence rates of 30-40% of balloon angioplasty a whole family of different angioplasty techniques has been developed since then. Among those are atherectomy devices, laser angioplasty, radiofrequency angioplasty, high and low speed rotational angioplasty and stenting of stenosed vessels. Balloon angioplasty has been extended to aortic and mitral valve stenoses, and supraventricular and ventricular tachycardias can now be treated by catherter ablation techniques. In 1989 an International Symposium on standard and newer inter ventional techniques has been held at the University of Ulm. This volume contains the essential parts and presentations of the international faculty of experts in the field. In four chapters the principles, advantages, pitfalls and future developments of coronary angiopiasty, angioplasty of peripheral arteries, balloon valvuloplasty and of catheter ablation of supraventricular and ventricular tachycardias are described in detail.
