Résumé : l'objectif de cette étude est d'évaluer la sensibilité de deux échelles de dépistage, le BEARNI et le MoCA, dans la détection de troubles cognitifs chez des sujets consommateurs chroniques de cannabis. Nous avons comparé les performances de consommateurs chroniques de cannabis et de volontaires sains au score total et aux sous-tests du MoCA et du BEARNI. Les résultats ne montrent pas de différence dans les scores totaux du MoCA et du BEARNI entre les deux groupes. Cependant, le BEARNI se différencie significativement du MoCA, dans le dépistage des troubles de la fluence verbale chez les sujets consommateurs de cannabis. Néanmoins, l'ensemble de nos sujets ont un niveau d'étude supérieur, ces résultats sont préliminaires et restent à confirmer. La poursuite du recrutement de sujets ainsi que le recrutement de sujets de tous niveaux d'étude permettront de mieux préciser quels sous-tests pourraient être efficaces dans le dépistage des troubles neurocognitifs induits par la consommation chronique de cannabis.

En France, le cannabis est la drogue illégale la plus répandue chez les jeunes. L'âge moyen de la première consommation se situe à 15,3 ans. Le jeune consommateur s'expose à des risques somatiques, psychologiques et sociaux souvent méconnus. Toutefois peu de médecins généralistes pensent devoir s'impliquer dans la prise en charge des usagers de cannabis. Une revue de littérature a été effectuée pour faire le bilan des connaissances sur les risques liés à l'usage de cannabis chez l'adolescent et mettre en avant les outils d'aide à leur prise en charge au cabinet de médecine générale. L'objectif de cette étude est de permettre de consolider et d'étayer les notions actuelles afin de pouvoir prendre en charge de manière optimale l'adolescent consommateur. Les études récentes montrent que, chez cette population, le cannabis agit en altérant la maturation cérébrale. En effet, la consommation induit l'apparition de troubles psychologiques tels que des troubles anxieux pouvant amener à la dépression. On lui trouve également une responsabilité dans certains troubles cognitifs tels que la dégradation des fonctions exécutives et de la mémoire avec un effet-dose prouvé surtout si l'exposition survient avant 15 ans, pouvant ainsi induire un échec scolaire. Des recommandations existent, basées sur le repérage précoce, l'évaluation et l'intervention brève. Des outils standardisés sont à disposition du médecin généraliste afin de dépister et d'évaluer les consommations. L'entretien motivationnel, l'intervention brève, les thérapies systémiques sont autant de thérapies ayant fait leurs preuves dans la prise en charge des jeunes consommateurs. La littérature nous montre que la consommation de cannabis chez l'adolescent est une problématique de santé publique qui doit interpeller le médecin généraliste dans sa pratique et l'inciter à repérer les sujets à risque. La prise en charge de l'adolescent usager reste complexe et nécessite une approche globale psycho-sociale et familiale.

ADHD is a highly comorbid disorder in patients with substance use disorders (SUD). This comorbid patient population is, however, often undertreated. Diagnosing ADHD in patients with SUD is not easy due to the large overlap of the ADHD characteristics with symptoms of intoxication and withdrawal. These patients often disappear prematurely from the treatment services, on the one hand because their untreated ADHD symptoms make it more diffi cult to follow the therapy, on the other hand because the treatment programs do not take their limitations into account.
