Aterosklerosis adalah sumber utama morbiditas dan mortalitas di negara maju. masalah ini semakin besar tiap tahunnya, bahkan aterosklerosis dianggap telah merenggut kehidupan lebih dari semua jenis kanker dan menimbulkan kerugian materi yang cukup besar. aterosklerosis adalah perubahan dinding arteri yang ditandai adanya akumulasi lipid ekstra sel,rekrutmen dan akumulasi lekosit, pembentukan sel busa, migrasi dan proliferasi miosit, deposit matrik ekstra sel (misalnya: kolagen, kalsium), akibat pemicuan berbagai multifaktor patogenesis yang bersifat kronik progresif, fokal atau difus, yang bermanifestasi akut maupun kronis, serta menimbulkan penebalan dan kekakuan arteri. Aterosklerosis dapat menyebabkan iskemia dan infark jantung, stroke, hipertensi renovaskular dan penyakit oklusi tungkai bawah tergantung pembuluh darh yang terkena. Proses pembentukan plak aterosklerosis memerlukan interaksi berbagai sitokin dan faktor pertumbuhan. Melibatkan sel-sel radang dan berbagai mediator kimia. Stres oksidatif akan menyebabkan gangguan vasodilatasi dan merupakan keadaan prokoagulan juga mengaktifkan sistem renin-angiotensin (RAS) dengan cara Angiotensin II (Ang II) yang ditangkap oleh reseptor Angiotensin II tipe 1 (AT1-R) yang menyebabkan ekspresi ROS. Pembahasan lebih mendalam tertuang dalam bab 1 buku ini. Radikal bebas merupakan salah satu faktor pemicu dalam penyakit kardiovaskular radikal bebas dapat berupa ROS (Reactive Oxygen Species) dan RNS (Reactive Nitrogen Species). ROS diproduksi oleh semua lapisan termasuk endotel, otot polos dan bagian adventisia. Peningkatan produksi ROS merupakan penyebab utama dalam disfungsi endotel pada aterosklerosis. Apa peranan CEC dengan aterosklerosis? Peningkatan jumlah CEC yang terdapat dalam darah pasien dengan berbagai macam penyakit terkait dengan proses cedera vaskular. Kehadiran mereka menggambarkan keparahan rusaknya pembuluh darah, fitur lain untuk tes fungsi pembuluh darah (plasma marker seperti faktor von Willebrand, flow mediated dilation) tidak dapat memberikan hasil dengan pasti. Jumlah CEC dapat memberikan informasi yang penting dalam pemantauan aktivitas penyakit dan atau efektivitas pengobatan. Peranan CEC dalam patogenensis aterosklerosis dijelaskan lebih detail dalam buku ini.
