Bachelorarbeit aus dem Jahr 2014 im Fachbereich Medizin - Innere Medizin, Note: 1,3, Diploma Fachhochschule Nordhessen Berlin-Treptow, Sprache: Deutsch, Abstract: Die Historie der klinischen Sektion war lange Zeit eine Geschichte der Methodenvielfalt. Seit der Differenzierung in ihre moderne Form zu Beginn des 19. Jahrhunderts hatte sich die klinische Obduktion bis etwa Mitte des 20. Jahrhunderts als Begleiterin und Reflexionsfläche des medizinischen Erkenntnisstandes der jeweiligen Zeit etabliert und bewährt. In diesem Zeitraum bildete die Obduktion das Zentrum des Fachgebiets Pathologie und wurde inhaltlich an die sich mit dem medizinischen Erkenntnisfortschritt verändernden klinischen Fragestellungen immer wieder angepasst, indem alte, bis dahin bewährte Sektions- und Präparationsmethoden durch neuentwickelte Methoden bereichert oder verdrängt wurden. In den vergangenen etwa 50 Jahren verlagerte sich die Obduktion jedoch zunächst schleichend, später immer offener und schneller an den Rand des Fachgebiets. Die Bereicherung der Sektionsprozedur um neue Methoden nahm ab, seltene Verfahren gerieten mehr und mehr in Vergessenheit. Die einst existierende Vielfalt an Präparationsmethoden für das Schlüsselorgan Herz übersichtlich aufgearbeitet darzustellen und damit vor dem Vergessen zu bewahren, ist ein Ziel dieser Arbeit. Nach einigen grundlegenden Ausführungen zur klinischen Obduktion, zum historischen Hintergrund von Sektionsmethoden und zum Einfluss der gewählten Sektionsmethode auf die spätere Feinpräparation des Herzens werden die zugänglichen deutsch- und englischsprachigen Monografien der „klassischen“ Sektionsmethoden ausgewertet in Bezug auf ihre jeweilige präparatorische Darstellung der einzelnen relevanten anatomischen Strukturen des Herzens (Konfiguration von Vorhöfen und Ventrikeln, Koronararterien, Myokard, Klappen, Reizleitungssystem, Maßerhebung). Die Schwerpunkte, Vor- und Nachteile sowie der historische Kontext der einzelnen Methoden sollen aufgezeigt und in Bezug auf ihre Praktikabilität beurteilt werden. Zusätzlich werden spezielle, separat publizierte Präparationsmethoden einzelner anatomischer Strukturen des Herzens dargestellt, ausgewertet und beurteilt, die aus verfügbarer unselbständiger Literatur stammen.

The brilliant yet simple idea of introducing a catheter percutaneously into an artery, without first dissecting it free, using a flexible guide wire, has led to a truly revolutionary breakthrough in abdominal x-ray diag nosis (SELDINGER, 1953). In the meantime, methods and techniques for injecting contrast media into various vessels have become largely standardized; innumerable publications have appeared which deal with every conceivable aspect of angiographic technique and interpretation. This volume is designed to present our experience with abdominal angiography. We deliberately refrained from any systematic discussion of the genitourinary tract, which has been adequately dealt with in the literature, also with respect to angiographic findings. Our interest in the retroperitoneal region is based mainly on its significance in differential diagnosis. In ten years of angiographic activity, our Department had made successful use of a simple technique which appears suitable also for smaller hospitals. We wish to point out its diagnostic potential and, at the same time, to outline its limitations. Our experience embraces 2804 abdominal angiograms, which we have classified according to clinical and morphologic anatomical criteria. Their diagnostic interpretation has been compared with the surgical or histopathological results. This may help others to avoid errors of the type which we discovered in our own work. Angiographic diagnosis requires not only familiarity with normal radiographic anatomy, but also specific knowledge of angiographic patho morphology. We have tried to identify those features which typify the individual findings and to derive therefrom valid generalizations with the aid of simple sketches.

Hier steht die Chemothera pie im Vordergrund. 1m Hinblick auf die Frage einer Hormonbehandlung kommt neuerdings auch dem Rezeptornachweis im Tumorgewebe Bedeu tung zu.
