Ein geschlossenes psychiatrisches Setting bedeutet Zwang: Besonders in der Akutversorgung werden Zwangsmaßnahmen durchgeführt. Trotz grundlegender Veränderungen durch Reformen der letzten Jahrzehnte in den entwickelten Ländern bezüglich der Behandlung von Patienten ist Zwang in der deutschen Psychiatrie noch immer weit verbreitet. Nach aktueller Studienlage sind Patienten auf geschlossenen psychiatrischen Akutstationen besonders stark von Zwangsmaßnahmen und Zwangsmedikation betroffen, obwohl jede Zwangsbehandlung sowie jede Zwangsmaßnahme einen gravierenden Grundrechtseingriff darstellt. In der psychiatrischen Praxis scheint das Bewusstsein dafür nicht durchgängig vorhanden zu sein und so gibt es noch immer Mängel sowie einen großen Handlungsbedarf bezüglich dieses ethisch brisanten Themas. Vor diesem Hintergrund ist ein Umdenken entstanden, das sich dadurch auszeichnet, Zwang, Türschließung, Restriktion und Isolation in der Behandlung psychiatrisch erkrankter Menschen zu reduzieren Eine konkrete Lösung der Problemstellung könnte das Konzept der „offenen Tür“ in der Psychiatrie sein, welches in den letzten Jahren oft diskutiert wurde und in bereits mindestens zwanzig (von insgesamt ca. 400) psychiatrischen Kliniken in Deutschland praktiziert wird. Darüber hinaus spielt eine frühzeitige Wiedereingliederung in die Gemeinschaft eine tragende Rolle. Das bedeutet beispielsweise, dass die Betroffenen schon während des Klinikaufenthaltes Zugang zu gemeindeintegrierten, ambulanten und komplementären Angeboten erhalten oder versucht wird, akutpsychiatrische Patienten generell länger im ambulanten und häuslichen Setting zu behandeln und zu versorgen, anstatt die Betroffenen stationär zu isolieren. Die Psychiatrie steht deshalb auch in Zukunft vor großen Aufgaben und Herausforderungen, um sich als ein modernes medizinisches Fach zu positionieren, zu entstigmatisieren und um die Selbstbestimmungsrechte und Autonomie der Patienten zu wahren.

Trotz grundlegender Reformen bei der Behandlung von Patienten ist Zwang in der deutschen Psychiatrie noch immer weit verbreitet. Jede Zwangsbehandlung sowie jede Zwangsmaßnahme stellt dabei einen gravierenden Grundrechtseingriff dar. In einer Stellungnahme der zentralen Ethikkommission der Deutschen Ärztekammer hat Wiesing im 'Deutschen Ärzteblatt' festgestellt, dass dieses Bewusstsein bei den Fachkräften in der Psychiatrie nicht immer vorhanden zu sein scheint. Das Statement und die ethische Brisanz dieses Themas zeigen Mängel sowie den großen Handlungsbedarf in der psychiatrischen Praxis. Vor diesem Hintergrund entstand ein Umdenken hin zu einer Psychiatrie, die Zwang, Türschließung, Restriktion und Isolation in der Behandlung psychiatrisch erkrankter Menschen so weit wie möglich reduziert. Das Konzept "Offenen Tür" wird bereits in mindestens zwanzig psychiatrischen Kliniken in Deutschland praktiziert und gilt dennoch mancherorts noch als unrealistische Vision. In dieser Publikation stellt der Autor die Öffnung von psychiatrischen Abteilung im Krankenhaus und einhergehende Konzepte auf institutioneller und organisatorischer Ebene in der akutpsychiatrischen Regelversorgung dar und vergleicht diese. Außerdem untersucht er in diesen Zusammenhang, inwieweit diese Ansätze dazu beitragen können, Zwangsmaßnahmen und -behandlungen von Patienten zu reduzieren. Darüber hinaus schließt der Autor rechtliche Fragestellungen ein, die sich in psychiatrische Einrichtungen mit offenen Eingangstüren stellen. The revisions of both DSM-IV and ICD-10 have again focused the interest of the field of psychiatry and clinical psychology on the issue of nosology. This interest has been further heightened by a series of controversies associated with the development of DSM-5 including the fate of proposed revisions of the personality disorders, bereavement, and the autism spectrum. Major debate arose within the DSM process about the criteria for changing criteria, leading to the creation of first the Scientific Review Committee and then a series of other oversight committees which weighed in on the final debates on the most controversial proposed additions to DSM-5, providing important influences on the final decisions. Contained within these debates were a range of conceptual and philosophical issues. Some of these - such as the definition of mental disorder or the problems of psychiatric “epidemics” - have been with the field for a long time. Others - the concept of epistemic iteration as a framework for the introduction of nosologic change - are quite new. This book reviews issues within psychiatric nosology from clinical, historical and particularly philosophical perspectives.
