Good health is widely believed to be intimately associated with economic growth and development. A number of governments of developing countries have been combating poverty and boosting development through providing adequate access to health care. However, developing countries have been constrained by the limited governmental revenue. Thus, additional resources should be mobilized through appropriate instruments. Health insurance is a highly practical instrument. However, health insurance in rural areas is problematic and the demand for private health insurance and community-based health insurance scheme is inadequate. The China Health Insurance Experiment (CHIE) was designed to test insurance as an alternative for health care financing in rural China. Six experimental groups were assigned to different insurance plans in 1989; each group switched to a new plan in 1990. The insurance plans differed according to coinsurance rates. In this study, data from 7,333 panel sample enrollees were used to estimate outpatient and inpatient demand functions for episodes of care. Demand functions were estimated using negative binomial regression (NBR); coinsurance elasticities were also estimated using first difference (fixed effects) models to reduce bias from unobserved heterogeneity. The principal findings are: (1) in rural China coinsurance exerts a significant and negative effect on demand; (2) coinsurance effects are similar for different population sub-groups and for different types of episodes; (3) elasticities of demand varied according to access to care conditions; (4) patterns of socio-demographic effects were similar to those reported on North American populations. These results provide support for proponents of health insurance as a primary means of financing rural health services in low-income developing countries.

Especially since the 2003 SARS crisis, China’s healthcare system has become a growing source of concern, both for citizens and the Chinese government. China’s once praised public health services have deteriorated into a system driven by economic constraints, in which poor people often fail to get access, and middle-income households risk to be dragged into poverty by the rising costs of care. The New Rural Co-operative Medical System (NRCMS) was introduced to counter these tendencies and constitutes the main system of public health insurance in China today. This book outlines the nature of the system, traces the processes of its enactment and implementation, and discusses its strengths and weaknesses. It argues that the contested nature of the fields of health policy and social security has long been overlooked, and reinterprets the NRCMS as a compromise between opposing political interests. Furthermore, it argues that structural institutional misfits facilitate fiscal imbalances and a culture of non-compliance in local health policy, which distort the outcomes of the implementation and limit the effectiveness of insurance. These dynamics also raise fundamental questions regarding the effectiveness of other areas of the comprehensive New Health Reform, which China has initiated to overhaul its healthcare system.

Abstract: Background The transition to a market-oriented economy in China has created huge economic growth in the recent two decades. However, the health care system was transformed to the one that residents cannot afford health care services and some households are driven to poverty. This is mainly because of the rapid medical expense inflation and imbalanced health resources. The health insurance schemes, on the other hand, contribute to the accessible health care and financial part of individuals' out-of-pocket spending. The promotion of health insurance coverage, with strong association on access to health care, is crucial to utilization of health services under the current health care system. Methods This systematic review was conducted by electronic database searching PubMed, Google Scholar and CNKI published in recent 15 years. The keywords of "health insurance" OR "insurance scheme" AND "access to health care" OR "health care utilization" AND "urban China" OR "rural China" were used. 12 observational studies were identified and included in the systematic review. The STROBE Guideline was adopted for the quality assessment among these studies. Results Of the12 observational studies included in the systematic review, these studies were mainly focusing on New Cooperative Medical Scheme (NCMS) and Urban Resident Basic Medical Insurance (URBMI) Scheme and the relation with access to health care in recent decade in China. Household survey-based designs were the most commonly used method. Based on the results conducted in the studies, a preliminary outcome showed that health insurance enrollees had a positive effect on health care utilization. Both the NCMS and URBMI schemes have increased the utilization in health care, with different percentage, and the accessibility because of financial assistant and subsidization. Conclusion The health insurance schemes, both for NCMS and URBMI, have increased inpatient and outpatient utilization. Additionally, the schemes have reduced the cost of delivery, but it does not reduce the out-of-pocket payments on seeking health care in some regions. Lastly, policy implications and recommendations were made on the demand and supply sides in health care, as well as the financial protection regarding to the schemes. This study investigates the situation of Universal Health Care (UHC) in China from a health economic perspective. The first chapter introduces the historical background, analyzes the relevance of UHC and sheds light on the current health insurance status. In this chapter a new holistic health insurance theory is proposed that allows the inclusion of preventive medicine. The second chapter introduces the "Definition and concept" consisting of three dimensions: Firstly, the height dimension with the leading question "What proportion of the costs is covered?". Secondly, the depth dimension that is concerned with the question "Which benefits are covered?". This chapter puts a special focus on the important economic role of non-communicable diseases. Thirdly, the breadth dimension which investigates the question "Who is insured?". The third chapter, looking at the first dimension, found a high but shrinking amount of out-of-pocket payments and catastrophic health payments. Comparing the payment and benefit distributions, it found the ability to pay principle and insufficient separation of health service payments from its consumption. The second dimension discovered problems concerning the roles of ministries, financing and the benefit package. Reforming these areas will be necessary to provide people with appropriate health care. The third dimension showed that migrant workers are exposed to more health risks, have less access to health care and a lower health status. The de facto coverage rate for the Chinese population (including migrant workers) was calculated to be 81.19% in 2011 and 82.16% in 2020. The goals of the Chinese Communist Party (90% in 2011 and nearly 100% in 2020) are hence not reached.
