The author hypothesizes that the defense of denial can serve an adaptive function for family members not only immediately after the tragedy occurs, but for many years. the author interviewed nine family members from five different families, whose close relative had suffered a severe closed head injury six to eight years previously. the commonalities were the ages of the injured members, the severity of the injury, and the devotion and loyalty of the family members for their injured relative. After reviewing the literature on denial, and head injury, and interviewing the families in depth, the author analyzed the results of the interviews, and compared the results with the existing literature. Despite pessimistic prognoses all except two family members believed their relative would have a complete recovery. In retrospect, those who had been in a state of strong denial attest to the fact that their denial was helpful. the one parent not in denial experienced depression and hopelessness. Siblings not in denial distanced themselves from their injured sibling, avoided the pain of the situation. the unique circumstances of traumatic brain injury--the uncertain outcome of neurological injury, the "partial death" of the victim, the extended period of mourning--provide fertile soil for the growth of optimism and hope which are supported by the denial. Through the years denial seems cyclical, ebbing and flowing with the emergence of new experiences. A sense of reality gradually replaced omnipotent feelings and myths of complete recoveries. A variety of experiences brought about the gradual erosion of denial. Families seemed to have the least difficulty adjusting to the injured victim when the personality changes were minimal. Six to eight years post-accident all family members interviewed possessed realistic goals for their injured family member. Hope was still alive amongst the family members, who refused to accept the idea that there might be an end to progress for their relative. Denial and an acceptance of reality seem able to co-exist in a positive way, with denial present for years, still helping a family adapt.
