Deployment of forces in hostile or unfamiliar environments is inherently risky. The changing missions and increasing use of U.S. forces around the globe in operations other than battle call for greater attention to threats of non-battle-related health problemsâ€"including infections, pathogen- and vector-borne diseases, exposure to toxicants, and psychological and physical stressâ€"all of which must be avoided or treated differently from battle casualties. The likelihood of exposure to chemical and biological weapons adds to the array of tactical threats against which protection is required. The health consequences of physical and psychological stress, by themselves or through interaction with other threats, are also increasingly recognized. In addition, the military's responsibility in examining potential health and safety risks to its troops is increasing, and the spectrum of health concerns is broadening, from acute illness and injury due to pathogens and accidents to possible influences of low-level chemical exposures, which can manifest themselves in reproductive health and chronic illnesses years later, perhaps even after cessation of military service. Strategies to Protect the Health of Deployed U.S. Forces develops an analytical framework for assessing risks, which would encompass the risks of adversed health effects from battle injuries, including those from chemical- and biological-warfare agents, and non-battle-related health problems. The presumed spectrum of deployment ranged from peacekeeping to full-scale conflict.

Because of its strong interest in providing airmen with the cross-cultural skills that have grown ever more essential to successful mission accomplishment in foreign environments, the Air Force asked RAND to provide a foundation for the design of a comprehensive Air Force program of cross-cultural training and education. RAND researchers responded by first creating a taxonomy covering all behaviors relevant to cross-cultural performance after the need for such a taxonomy became evident from a review of the literature on cross-cultural performance and discussions with Air Force personnel. From this taxonomy, the researchers developed a framework of 14 categories of cross-cultural behaviors--nine categories of enabling behaviors and five of goal-oriented behaviors. This framework was then used in designing a survey for 21,000 recently deployed airmen that asked them to rate the importance of the behaviors to their deployed performance and the helpfulness of training they had received in the behaviors (both over their careers and just prior to deployment). Respondents were also asked to indicate how much training they had received. Recommendations and suggestions for the design of a comprehensive program of cross-cultural training and education and for further research steps were made based on extensive analyses of the results, which included determining whether training needs differed by AFSC, grade (enlisted/officer), and deployment location.

To guide mission planning, military decision makers need information on the health risks of potential exposures to individual soldiers and their potential impact on mission operations. To help with the assessment of chemical hazards, the U.S. Army Center for Health Promotion and Preventive Medicine developed three technical guides for characterizing chemicals in terms of their risks to the mission and to the health of the force. The report reviews these guides for their scientific validity and conformance with current risk-assessment practices. The report finds that the military exposure guidelines are appropriate (with some modification) for providing force health protection, but that for assessing mission risk, a new set of exposure guidelines is needed that predict concentrations at which health effects would degrade the performance of enough soldiers to hinder mission accomplishment.
