(DOD) has expanded its involvement in overseas military operations, it has grown increasingly reliant on its federal civilian workforce to support contingency operations. GAO was asked to discuss DOD's (1) force health protection and surveillance policies, (2) medical treatment policies that cover federal civilians while they are deployed to support contingency operations in Afghanistan and Iraq, and (3) differences in special pays and benefits provided to DOD's deployed federal civilian and military personnel. For this statement, GAO primarily drew on its September 2006 report that addressed these objectives. For its report, GAO analyzed over 3,400 deployment-related records at eight component locations for deployed federal civilians and policies related to defense health care, reviewed claims filed under the Federal Employees' Compensation Act (FECA); and examined major provisions of special pays and disability and death benefits provided to DOD's deployed federal civilians and military personnel.

GAO recommended that DOD establish an oversight and quality assurance mechanism to ensure that all components fully comply with its requirements. In 2007, DOD issued a new force health protection and surveillance policy that if effectively implemented will establish such a mechanism.

The Senate Armed Services Committee required GAO to examine DOD's policies concerning the health care for DOD civilians who deploy in support of contingency operations in Afghanistan and Iraq. GAO analyzed over 3,400 deployment-related records for deployed federal civilians and interviewed department officials to determine the extent to which DOD has established and the military services and defense agencies (hereafter referred to as DOD components) have implemented (1) force health protection and surveillance policies and (2) medical treatment policies and procedures for its deployed federal civilians. GAO also examined the differences in special pays and benefits provided to DOD's deployed federal civilians and military personnel.

The President has declared that the United States must be prepared to assist in the stabilization and reconstruction of at-risk countries and regions to promote the security of the nation. Reconstruction and stabilization operations require a "whole of government" approach with the deployment of both uniformed and civilian personnel from the Department of Defense (DoD) and other departments and agencies. To support these stabilization and reconstruction missions, the President has requested $249.0 million in the fiscal year 2009 budget to build an Active and Standby Response Corps of over 2,000 federal civilian personnel from all 15 civilian agencies and to build a Civilian Reserve Corps comprised of about 2,000 experts from state and local governments and from the private sector. H.R. 1084, which passed in the House on March 5, 2008, authorizes funds and responsibilities for the Office of the Coordinator for Reconstruction and Stabilization within the Department of State to establish and manage these corps. In a recently issued memorandum and policy, the Under Secretary of Defense (Personnel and Readiness) underscored the need to increase the capacity of DoD civilian volunteers to support these and other contingency operations. The memo highlights the current need to support manning of Provincial Reconstruction Teams (PRTs) and Ministerial Defense Teams (MDTs) as examples of reasons for building civilian deployment capacity. With regard to the federal civilian employees that have been or will be deployed to Iraq or Afghanistan, the Subcommittee on Oversight & Investigations, Committee on Armed Services, U.S. House of Representatives, has highlighted the following key areas for follow-on actions and further inquiry and investigation: medical care policies vs. practices, the role of the Department of Labor Office of Workers' Compensation Programs, mental health and stress disorders, and equitable and sufficient incentives and benefits.
