What's new in antidepressant research? Despite the availability of antidepressants for over 40 years, a substantial proportion of depressed patients do not respond adequately to treatment. In 1988, the Deparunent of Psychology at San Diego State University initiated the first in a planned conference series on Contemporary Issues in Clinical Psychology. It was decided that the focus of this first conference would be depression. Consequently, a number of distinguished scholars were invited to San Diego to discuss contemporary theoretical, empirical, and treatment issues in depressive disorders. This volume contains the results of this conference. Each chapter remains true to the original presentation, although each has been extensively reworked by the authors for inclusion in a book format, and in some cases co-authors have aided in revisions for the volume. Given the sheer quantity and impressive quality of contemporary research, it may not be possible to overstate the impact of psychological approaches on our understanding of depressive disorders. Accordingly, the aim of this conference was, within the limited amount of time available for such an endeavor, to chronicle the current status of the psychology of depression. In inviting participants to this forum, no attempt was made to reflect only certain theoretical views. Contemporary psychological theory and research in depression, however, are dominated by cognitive viewpoints, and the influence of cognitive perspectives is thus unmistakable throughout the present volume.

For many years, we have taught clinical psychopharmacology to residents in psychiatry, neurology, and medicine, as well as to medical students, nurses, psychologists, counselors, and social workers. We have also maintained active clinical practices and research programs. It is our hope that these experiences have produced a book that will serve as a resource in antidepressant therapy for clinicians from many different disciplines. The purpose of Pharmacotherapy of Depression is to provide a compreh- sive overview of antidepressant therapy for clinicians. The contents have evolved from our years of seminars, lectures, and case discussions with our trainees and from our continuing medical education programs. We have tried to provide sufficient depth in reviews of the research literature to support clinical rec- mendations without burdening the reader with information that has little r- evance to the clinical use of antidepressants. We are indebted to the outstanding clinician scientists who have contributed to this volume and to our students who have guided us in its development. We hope that the reader will find that the final product represents a solid foundation for clinicians who are, or will be, prescribing antidepressants. It comes very close to replicating the formal didactics that our trainees experience. Although in practice we supplement this with intensive supervision of patient management, we hope that Pharmacotherapy of Depression will stand alone as a guide to the use of medications for the treatment of depressive illness.

The last two decades have seen unprecedented increases in health care costs and, at the same time, encouraging progress in psychotherapy research. On the one hand, accountability, cost-effectiveness, and efficiency have now become commonplace terms for providers of mental health services whereas, on the other hand, an increasingly voluminous literature has emerged supporting the effectiveness of a number of types of psychotherapies. There now exists the possibility for the design and delivery of mental health services that-drawing upon this literature-more closely approximate empirically established data concerning the appropriateness and effectiveness of psychotherapy. The Handbook of the Brief Psychotherapies is intended to capture one major thrust of this movement: the development of a group of empirically grounded, time-limited therapies all sharing a common interest in the clinical utilization of a structured focus and an emphasis on time and action. For many years, professional self-interest, competing theoretical para digms, and the vagaries of practice, wisdom, and clinical myth have influenced the practice of psychotherapy. A critical questioning of the resulting, predomi nantly nondirective, open-ended, and global therapies has led to a growing emphasis on action-oriented, problem-focused, time-limited therapies. Yet, ironically, this interest in the brief psychotherapies has not so much involved a radical departure from traditional therapeutic modalities as it has emphasized a new pragmatism about how time, action, and structure operate in life as well as in therapy.
