Since the adoption of the mcgraw rubber ligature in surgery, one of the chief oh stacles to a finished technique in its use has been the fact that the ligature always made the orifice in the tissues much too large. The thickness of the ligature itself, even when under tension, and the fact that it had to be doubled at the eye of the needle were the two chief factors in this trouble. The style of needle which we represent, the design of Dr. Angus mclean, of De troit, Mich successfully overcomes the difficulty. It is so simple that its advan tages must be self-evident, and yet it perfects the technique in the use of rubber ligatures. A slot in the eye provides for the easy introduction of the ligature into the butt of the needle; there is no threading necessary. When the ligature is intro ducea into the eye, much after the manner of the introduction of thread into the blind man's needle, it is cut off near the eye and a ferule which closely fits the needle is pushed snugly over the ligature and the eye, securely fastening it. In this way the thickness of the needle at the eye end is increased only by the thickness of the ferule, which is inconsiderable, and the surgeon has an 0p portunity for using a rubber ligature without appreciable increase of diameter at its point of entrance into the needle - something which we believe has not heretofore been accomplished in work with ligatures of this type. Excerpt from Detroit Medical Journal, Vol. 2: December, 1902 Easy To Try. - The Family Doctor, a London publication, reports the following case: A young girl for four days without cessation from singultus, the attack being due, apparently, to some gastric disorder. When she put out her tongue for a few seconds it was found the hiccough ceased. She was then ordered to stick out this member at regular intervals for a few minutes, at the termination of which only a few slight spasms followed. 'she was then ordered to repeat, when the singul tus ceased altogether, and did not again return. It, therefore, would seem to be proper to try continuous or rhythmic traction of the tongue in these cases. Upon this point there has been considerable change of opinion of late, and justly so. The method at present practiced in Detroit is as follows We remove the patient to the small-pox hospital at once, vaccinate all inmates of the house, disinfect the rooms and their contents, including the clothing of all persons living in the house, thoroughly with formaldehyde, disinfect the persons themselves by means of a bichloride of mercury bath, paying special attention to the hair, and then open the house and do not quarantine. We then keep all persons who have been directly exposed in the house from which the patient was removed under daily observation for a period of two weeks so that any possible new cases may be promptly removed.
