Those who do not feel pain seldom think that it is felt. SAMUEL JoHNSON, The Rambler, no. 48 (September 1, 1750) Who among us has not experienced the suffering of a patient with chronic disease, who in addition to the vicissitudes of fatigue, anxiety, and frustration, must also deal with the suffering of pain? Who among us has not considered, and then reconsidered, whether a patient's complaints are worthy of a narcotic and thence worried about the social and legal implications of chronic use? Who among us has not refused pain medications to our patients for fear that use was turning into abuse? Finally, who among us would not have liked a clinical guide to a myriad of syndromes, all of which have pain as their common denominator, in the hopes of developing some strategy to prioritize treatment. Our purpose in preparing The Pain Management Handbook is to provide the informa tion needed by clinicians to develop strategies that optimize pain management. It is the goal of the editors and authors that the present handbook, above all else, will be clinically useful. Its aim is to provide practical information regarding the diagnosis and treatment of disorders causing pain, along with tables and graphics to provide the busy practitioner with rapid access to relevant data.

The full array of drug treatment and analgesic strategies is discussed in detail, and often-conveyed through a detailed management algorithm in each chapter. This edition of the companion volumes Muscle Pain: Understanding the Mech- isms and Muscle Pain: Diagnosis and Treatment is essential reading for those interested in clinical approaches to acute and chronic pain conditions involving muscle tissues and in the mechanisms underlying these conditions. The volumes cover a very important topic in pain medicine, since muscle pain is very common and can often be dif?cult to diagnose and treat effectively. Furthermore, chronic pain involving muscle and other components of the musculoskeletal system increases with age, such that it is a common complaint of those of us who are middle-aged or older. Indeed, as changing population demographics in “west- nized” countries result in higher proportions of the population living longer and being middle-aged and elderly, chronic muscle pain will likely become even more of a health problem. In the case of acute muscle pain, this can often be very intense, and in the short term can limit or modify the use of components of the musculoskeletal system associated with the sensitive muscle. Chronic muscle pain can also be intense, as well as unpleasant and disabling, and it is in many cases the over-riding symptom of most musculoskeletal disorders that are associated with long-term deleterious changes in musculoskeletal function.

Breakthrough pain (BTP) is episodic pain that emerges through the treatment of otherwise well-managed chronic background pain. Often called pain flare or transient pain, BTP negatively affects the function and quality of life of the patient and often results in a number of other physical, psychological and social problems. Breakthrough pain is a common occurrence affecting approximately two-thirds of the estimated 50 to 100 million chronic pain sufferers in the US. It can have multiple causes with various pathophysiologies, and can present with numerous clinical features and complications. The clinical features vary from individual to individual, and may vary within an individual over time. The successful management of breakthrough pain depends on proper assessment, treatment, and reassessment. Inadequate assessment can lead to ineffective or inappropriate treatment. Similarly, inadequate reassessment may lead to continuance of ineffective or even harmful treatment. In recent years, the need to educate physicians about pain management has been garnering increased attention from prominent medical associations and the media. Despite ongoing efforts to improve pain treatment, however, the need persists for evidence-based educational materials for physicians in the area of pain diagnosis and management. Part of the Oxford American Pain Library, this highly practical guide covers current approaches and new developments in the assessment and management of Breakthrough Pain, including both cancer-related pain and non-cancer chronic pain. It addresses the roles of opioid and non-opioid pharmacotherapy and presents non-pharmacologic interventions, as well. This concise yet comprehensive reference on Breakthrough Pain is ideal for palliative care doctors, anesthesiologists, pain medicine specialists and oncologists, as well as for primary care physicians and internists on the frontlines of care.

Chronic pelvic pain (CPP) involves a list of deviations with persistent pain in the pelvic area as the main factor. Multiple pain syndromes exist for this, and, after assessment, the reason for the chronic symptoms often remains unclear. The exclusion of the origins or reasons for the pain that can be treated curatively is of the greatest importance. When no reasons for the pain can be found, or when the pain cannot be healed, symptomatic treatment will be necessary. The most successful treatment of CPP is a multidisciplinary approach, bringing together doctors, nurses, physical therapists, osteopaths, and psychologists. Chronic pain has an important impact on the quality of life and strong negative consequences on the psychologic and emotional state of the patient. In addition, the success of the (usually long-term) treatment is not easy to predict. Beginning with a general discussion, this book considers pain abnormalities in the most important parts of the small pelvis. It also discusses various treatments of these issues, and delves into the psychological approach to such pain.
