Galt es früher, den Patienten so schnell wie möglich in eine geeignete Zielklinik zu transportieren, rückt heute die medizinische Versorgung immer stärker in den Mittelpunkt und nimmt einen entscheidenden Anteil im Einsatzgeschehen ein. Mit der Einführung des Notfallsanitäters / der Notfallsanitäterin zu Beginn des Jahres 2014 haben die Entscheidungsträger konsequenterweise auf diesen Aufgabenwandel in der präklinischen Notfallversorgung reagiert. Bisher erhalten die Rettungsdienstorganisationen lediglich eine Vergütung für den Transport, jedoch nicht für die medizinische Versorgung, Behandlung und Betreuung von Notfallpatienten. Aus gesundheitspolitischer Betrachtung werden hierdurch falsche Anreize für die Rettungsdienstorganisationen gesetzt. Eine denkbare und potenzielle Abhilfe könnte die Einführung der sogenannten „Rettungsdienst Diagnosis Related Group“, kurz R-DRG sein.

Die bereits in der stationaren Versorgung etablierten German DRG bilden die ideale Grundlage fur die R-DRG. So wird auch bei den R-DRG der individuelle okonomische Aufwand am Patienten berucksichtigt. Ein grosser Vorteil dieser Finanzierungsform ist, dass sie die Grundkosten und somit die Wirtschaftlichkeit des Rettungsdienstes sicherstellt. Zusatzlich werden durch Belohnungs- bzw. Bestrafungssysteme neue Anreize fur alle Beteiligten geschaffen. So erhalten Rettungsdienstorganisationen eine Bonuszahlung, wenn sie bzw. als Ausbildungsbetrieb agieren oder die gesetzlich definierte Hilfsfrist einhalten. Trotz der starken landesrechtlichen Pragung des Rettungsdienstes konnte mit Hilfe der R-DRG eine bundesweite Finanzierung sichergestellt werde

The monetary system is the indispensable missing link in the debate of sustainability, and whether the current financial system can handle these evolved needs. To date, the UN Sustainable Development Goals (SDGs) primarily have been financed either through the private sector, through conventional public sector taxes and fees, or through philanthropic commitment. Assuming a need of 4 to 5 trillion dollars annually in the 10 to 15 years left to finance our future, these conventional sources of finance are insufficient in terms of both the scale and speed of funding required to finance our future. Furthermore, the inherent instability of our financial system forces the world community to focus first and foremost on repairing and stabilizing the existing system. The development of cryptocurrencies using distributed ledger technologies (mainly blockchain) has prompted leading central banks to study the potential application of this approach to independently create purchasing power. In this vein, this book offers a new approach, namely introducing a parallel electronic currency specifically designed to finance global common goods and provide the resources necessary to achieve the SDGs. Furthermore, this mechanism would have a stabilizing effect on the existing monetary system. The book argues that one way this could be achieved is by giving central banks a modified monetary mandate to inject new liquidity into the system using a top-down approach. Alternatively, liquidity could come from corporate or communal initiatives with crypto- or communal currencies in a bottom-up approach. The author maintains that by issuing a blockchain-enabled parallel electronic currency earmarked for SDG-related projects and using other channels for monetary flow rather than the conventional ones, the future could be financed in a different manner. In the long run, abandoning our current monetary monoculture and introducing a monetary ecosystem would stabilize international financial markets, increase monetary regulatory efforts, reduce negative externalities, create a social Pareto optimum and stabilize democracies. This book presents, in the same spirit as Fritjof Capra's The Tao of Physics, a Tao of finance--an outside-of-the-box approach to financing global common goods.

Kaniaris Experimental studies have been reported in which a prolonged action of steroid neuro­ muscular blocking agents followed bile acids administration(1, 2). A prolonged neuro­ muscular action of pancuronium has also been detected in patients with biliary obstruc­ tion(3). In the present study serum bile acids and intracellular liver enzymes were estimated after anaesthesia in which pancuronium was used as a muscle relaxant (Fig. 1). An at­ tempt is made to investigate the effect of pancuronium on bile acid levels. Material and Methods Twelve female patients aged between 40 and 50 years were studied. All of them were visited in the ward the night before the operation by the anaesthesiologist where their clinical condition was assessed and their consent was obtained to participate in the pre­ sent study. None of them had a history of hepatic disease or was taking drugs. The oper­ ation performed in all cases was modified radical mastectomy and intracellular liver en­ zymes are routinely determined in these patients preoperatively. The twelve patients were reassured and stated that they did not want to have any tranquillizer for premedi­ cation.
