INTRODUCTION Le médecin généraliste est régulièrement confronté à des pathologies dermatologiques, et il est parfois difficile d'y répondre au vu de la diversité sémiologique. Le site DERMATOLOGIC propose de mettre à disposition des médecins généralistes un outil d'aide informatique au diagnostic dermatologique, facile d'utilisation, gratuit, indépendant, adaptéà l'exercice ambulatoire et au profil des patients consultés, et ne traitant que des dermatoses les plus fréquemment rencontrées en médecine générale. MATERIEL ET METHODE Deux internes ont travaillé à la création de l'outil sous la direction d'un comité de pilotage composé de médecins généralistes enseignants de l'Université Paris Diderot. La partie création informatique a été confiée à des étudiants de l'école informatique HETIC. Ce partenariat n'a pas nécessité de financement. Le contenu scientifique a été élaboré à partir d'un ouvrage de référence en dermatologie et validé par un comité de relecture. RESULTAT L'outil propose de partir de la lésion élémentaire pour aboutir à un faisceau d'hypothèses diagnostiques, en fonction de plusieurs critères cliniques (topographie, couleur, délai d'apparition...) que l'utilisateur aura sélectionné. Chaque hypothèse diagnostique est associée à une fiche descriptive récapitulative avec photothèque, référence bibliographique et lien vers le collège des enseignants en ligne. CONCLUSION DERMATOLOGIC est un outil diagnostique informatique visant à améliorer la prise en charge des patients présentant les dermatoses les plus fréquentes en soins primaires, de manière simple, rapide et fiable. L'évaluation du site sur des cas cliniques n'a pas mis en évidence une supériorité de l'outil, mais ces résultats seront à comparer à une utilisation en situation réelle de consultation. DISCUSSION DERMATOLOGIC est le 1er outil d'aide diagnostique pour les pathologies cutanées à destination des médecins généralistes. Il regroupe les 50 dermatoses les plus fréquentes en soins primaires. Il pourrait être également un outil de formation initiale pour les étudiants peu formés à la dermatologie. Il est indispensable par la suite d'évaluer l'outil, sa place en consultation, son apport en pratique, et l'amélioration de la prise en charge des pathologies dermatologiques par les professionnels de soins primaires.
