Die Einführung der neuen Diagnose bringt sowohl Vor- als auch Nachteile mit sich. Gegen die Einführung spricht, dass sie Diagnose der komplexen PTBS einen unidirektionalen Zusammenhang zwischen dem traumatischen Ereignis und dem Symptom Emotionsregulation impliziert, wobei in einigen Fällen auch eine umgekehrte Ursache-Wirkungsbeziehung denkbar wäre. Ebenfalls besteht eine symptomatische Ähnlichkeit der komplexen PTBS mit der PTBS inklusive einer komorbiden Borderline Persönlichkeitsstörung (BPS). Bei genauerem Hinsehen ist allerdings zu dem Schluss zu kommen, dass sich die Symptome in ihren Ausprägungen ausreichend voneinander unterscheiden. Im Hinblick auf die Beurteilung des Klinischen Nutzens, welcher insbesondere bei dem ICD-10 als defizitär kritisiert wurde, ist zunächst festzustellen, dass die Unterteilung in klassische und komplexe PTBS mit einer Erhöhung der Komplexität des Diagnosesystems einhergeht. Eine hohe Komplexität geht dann mit einem niedrigen Klinischen Nutzen einher, wenn diese außerhalb des Forschungskontexts nicht notwendig ist. Jedoch lässt sich damit die Symptomatik einiger Patienten spezifischer und differenzierter abbilden, wodurch eine passgenauere Behandlung abgeleitet werden kann. Die Validität der Diagnose konnte durch statistische Verfahren wie der latent- profile analysis (LPA) und der konfirmatorischen Faktorenanalyse nachgewiesen werden. Als Therapiemaßnahme der komplexen posttraumatischen Belastungsstörung hat sich eine multimodale Behandlung etabliert, die innerhalb einer von der ISTSS veranlassten Expertenbefragung gewonnen wurde.
