In the same way that infrastructures such as transportation, electricity, sewage, and water supply are widely assumed to be integrators of urban spaces, information infrastructures are assumed to be integrators of information spaces. With the advent of Web 2.0 and new types of information infrastructures such as online social networks and smart mobile platforms, a more in-depth understanding of the various rights to access, use, develop, and modify information infrastructure resources is necessary. Perspectives and Implications for the Development of Information Infrastructures aims at addressing this need by offering a fresh new perspective on information infrastructure development. It achieves this by drawing on and adapting theory that was initially developed to study natural resource commons arrangements such as inshore fisheries, forests, irrigation systems, and pastures, while placing great emphasis on the complex problems and social dilemmas that often arise in the negotiations.

The United States does not have enough doctors. Every year since the 1950s, internationally trained and osteopathic medical graduates have been needed to fill residency positions because there are too few American-trained MDs. However, these international and osteopathic graduates have to significantly outperform their American MD counterparts to have the same likelihood of getting a residency position. And when they do, they often end up in lower-prestige training programs, while American-trained MDs tend to occupy elite training positions. Some programs are even fully segregated, accepting exclusively U.S. medical graduates or non-U.S. medical graduates, depending on the program’s prestige. How do international and osteopathic medical graduates end up so marginalized, and what allows U.S.-trained MDs to remain elite? Doctors’ Orders offers a groundbreaking examination of the construction and consequences of status distinctions between physicians before, during, and after residency training. Tania M. Jenkins spent years observing and interviewing American, international, and osteopathic medical residents in two hospitals to reveal the unspoken mechanisms that are taken for granted and that lead to hierarchies among supposed equals. She finds that the United States does not need formal policies to prioritize American-trained MDs. By relying on a system of informal beliefs and practices that equate status with merit and eclipse structural disadvantages, the profession convinces international and osteopathic graduates to participate in a system that subordinates them to American-trained MDs. Offering a rare ethnographic look at the inner workings of an elite profession, Doctors’ Orders sheds new light on the formation of informal status hierarchies and their significance for both doctors and patients.

A unique resource for the general public and students interested in immigration and public health, this book presents a comprehensive history of public health and draws 10 key lessons for current immigration and health policymakers. The period of 1820 to 1920 was one of mass migration to the United States from other nations of origin. This century-long period served to develop modern medicine with the acceptance of the germ theory of disease and the lessons learned from how immigration officials and doctors of the United States Marine Hospital Service (USMHS) confronted six major pandemic diseases: bubonic plague, cholera, influenza, smallpox, trachoma, and yellow fever. This book provides a narrative history that relates how immigration doctors of the USMHS developed devices and procedures that greatly influenced the development of public health. It illuminates the distinct links between immigration policy and public health policy and distinguishes ten key lessons learned nearly 100 years ago that are still relevant to coping with current public health policy issues. By re-examining the experiences of doctors at three U.S. immigration/quarantine stations—Angel Island, Ellis Island, and New Orleans—in the early 19th century through the early 20th century, Doctors at the Borders: Immigration and the Rise of Public Health analyzes the successes and failures of these medical practitioners' pioneering efforts to battle pandemic diseases and identifies how the hard-won knowledge from that relatively primitive period still informs how public health policy should be written today. Readers will understand how the USMHS doctors helped shape the very development of U.S. public health and modern scientific medicine, and see the need for international cooperation in the face of today's global threats of pandemic diseases.
