This book studies how doctors responded to - and helped shape - deep-seated fears about nervous degeneracy and population decline in France between 1750 and 1850. It uncovers a rich and far-ranging medical debate in which four generations of hygiene activists used biomedical science to transform the self, sexuality and community in order to regenerate a sick and decaying nation; a programme doctors labelled 'physical and moral hygiene'. Moreover, it is shown how doctors imparted biomedical ideas and language that allowed lay people to make sense of often bewildering socio-political changes, thereby giving them a sense of agency and control over these events. Combining a chronological and thematic approach, the six chapters in this book trace how doctors began their medical crusade during the middle of the Enlightenment, how this activism flowered during the French Revolution, and how they then revised their views during the period of post-revolutionary reaction. The study concludes by arguing that medicine acquired an unprecedented political, social and cultural position in French society, with doctors becoming the primary spokesmen for bourgeois values, and thus helped to define the new world that emerged from the post-revolutionary period.

Divine Doctors and Dreadful Distempers examines the discourse of seventeenth-century English physicians to demonstrate that physicians utilized cultural attitudes and beliefs to create medical theory. They meshed moralism with medicine to self-fashion an image of themselves as knowledgeable health experts whose education assured good judgment and sage advice, and whose interest in the health of their patients surpassed the peddling of a single nostrum to everyone. The combination of morality with medicine gave them the support of the influential godly in society because physicians’ theories about disease and its prevention supported contemporary concerns that sinfulness was rampant. Particularly disturbing to the godly were sins deemed most threatening to the social order: lasciviousness, ungodliness, and unruliness, all of which were most clearly and threateningly manifested in the urban poor. Physicians’ medical theories and suggestions for curbing some of the most feared and destructive diseases in the seventeenth century, most notably plague and syphilis, focused on reforming or incarcerating the sick and sinful poor. Doing so helped propel physicians to an elevated position in the hierarchy of healers competing for patients in seventeenth-century England.

We think we know what healers do: they build on patients’ irrational beliefs and treat them in a ‘symbolic’ way. If they get results, it’s thanks to their capacity to listen, rather than any influence on a clinical level. At the same time, we also think we know what modern medicine is: a highly technical and rational process, but one that scarcely listens to patients at all. In this book, ethnopsychiatrist Tobie Nathan and philosopher Isabelle Stengers argue that this commonly posed opposition between traditional and modern medicine is misleading. They show instead that healers are interesting precisely because they don’t listen to patients, using techniques of ‘divination’ rather than ‘diagnosis’. Healers construct genuine therapeutic strategies by identifying the origins of symptoms in external forces, outside of the mind of the sufferer. Modern medicine, for its part, is characterized by empiricism rather than rationality. What appears to be the pursuit of rationality is ultimately only a means to dismiss and exclude other forms of treatment. Blurring the distinctions between traditional and modern practices and drawing on perspectives from across the globe, this ethnopsychiatric manifesto encourages us to think in radically new ways about illness, challenging accepted notions on the relationship between sufferer and symptom.
