It is to the great and lasting credit of LORENZ BOHLER and his school that they have in the last decade developed and demonstrated so thoroughly the techniques for the conservative management of fractures. Nevertheless there have always been many, including some from BOHLER'S school, who have found considerable place for surgical management, and with the significant progress in general surgery seen in postwar years, a new stimulus has been given to this part of traumatic surgery, especially since bone injuries have become more complex and frequent. The concept of internal fixation is not new. The serious criticisms that have been levelled at it retain today their basic significance. Progress in the fields of asepsis, corrosion-free metal implants, operative experience and postoperative care has diminished the dangers but has not relieved the surgeon of responsibility. The Association for the Study of the Problems of Internal Fixation (AO) has devoted itself over a number of years to the basic principles and best methods of open treatment of fractures by means of extended clinical and scientific studies in order to determine in each individual case the most promising line of treatment. At the same time a well designed and tested instrument set has been developed with precise instructions for the appropriate techniques. As a result, the new observations about primary bone healing which have emerged from the practice of rigid internal fixation are as interesting as the uses to which they can be put in allowing early mobilization.

Es ist ein hohes und bleibendes Verdienst von LORENZ BÖHLER und seiner Schule, in den vergangenen Jahrzehnten die konservativen Behandlungsverfahren bei Knochen brüchen in umfassender Weise ausgearbeitet und dargestellt zu haben. Trotzdem hat es nie an Stimmen auch aus dieser Schule gefehlt, die dem operativen Vorgehen einen mehr oder weniger breiten Platz zuwiesen. Die Nachkriegszeit mit ihren bedeutenden Fort schritten für die allgemeine Chirurgie hat folgerichtig auch diesem Teil der Traumatologie neue Impulse gegeben, zumal durch die Zunahme und Kompliziertheit der Knochen verletzungen eine nur einseitige Einstellung den Aufgaben der Behandlung nicht mehr gerecht werden kann. Der Gedanke einer operativen Osteosynthese ist nicht neu Die ernsthafte Kritik, die an ihr geübt wurde, hat auch heute im Grunde noch ihre Bedeutung. Die erreichten Fort schritte auf dem Gebiet der Asepsis, der Verhütung von Infektionen, der Schaffung ein wandfreier metallischer Implantate, in der operativen Erfahrung und in der sachgemäßen Nachbehandlung haben zwar die Gefahren wesentlich vermindert, nicht aber die Verant wortung! Die Arbeitsgemeinschaft für Osteosynthesefragen hat sich nun in einer mehrjährigen Zusammenarbeit der Aufgabe gewidmet, durch ausgedehnte klinische und wissenschaft liche Prüfung die Grundlagen und die besten Methoden einer 0 per a t i v e Frakturbehand n lung herauszuarbeiten, um für den Einzelfall das erfolgversprechendste Verfahren wählen zu können. Hierzu wurde gleichzeitig ein wohldurchdachtes und erprobtes Instrumenta rium mit genauen Angaben zum technischen Vorgehen geschaffen. Dabei sind die, bei der stabilen Osteosynthese gewonnenen neuen, wissenschaftlichen Erkenntnisse über die "primäre Knochenbruchheilung" ebenso interessant, wie die vorteilhaften Aspekte für die funktionelle Nachbehandlung.

Because of the relatively low incidence of involvement efforts to enhance the diagnosis and management of elbow pathology are not common.
