Although dyspepsia has been investigated for a long period of time, there is no international agreement on what constitutes this condition nor any standardized guidelines. National guidelines followed by practitioners in different countries vary in diagnostic and therapeutic approach, underlining the necessity for a unique definition worldwide. Dyspepsia in Clinical Practice summarizes the current guidelines while offering a unified, practical definition of dyspepsia, and a diagnostic algorithm with an emphasis on the upper gastrointestinal endoscopy and rational first-line therapeutic approach based on epidemiology, pathophysiology, clinical presentation, diagnostic workup and response to previous therapy. Up-to-date scientific information about dyspepsia is presented from a practical, clinician's point of view. Written by experts in the field, this volume addresses dyspepsia in childhood and in the elderly, a very important issue often insufficiently emphasized in the literature. Guidelines are provided that can be easily followed in clinical practice, leading to a reduction in costs and increased patient safety. Dyspepsia in Clinical Practice will be of great value to gastroenterologists, internists, primary care physicians, pediatricians, infectious disease specialists, residents and fellows in training.

Dyspepsia is a disorder which affects a large portion of the population. However, it is very important to differentiate between commonly presenting symptoms like gastroesophageal reflux disease, functional dyspepsia, and irritable bowel syndrome. Also various classifications and treatment protocols have been developed and revised to provide a rational platform for the clinicians. The ROME III classification has to a large extent given a direction to the clinicians to base their practice on, however, is a classification based on clinical trials and is really helpful in daily practice. In this issue we have tried to focus on the need for classification and approach to the patient with dyspepsia.

Excerpt from On the Diagnosis and Treatment of the Varieties of Dyspepsia: Considered in Relation to the Pathological Origin of the Different Forms of Indigestion IN spite of the numerous works which have of late appeared on the diseases of the Stomach, I have constantly found, while engaged in the practical instruction of students, that they experience greater difficulties in the diagnosis of the different forms of Dyspepsia, and in the application of the appropriate remedies for these disorders, than in the treatment of almost any other class of diseases. The reason for much of the uncertainty which has attended this branch of medical practice, has appeared to me to lie in the fact that the various derangements of the digestion are so universally regarded as being of a purely functional nature and although many recent authors have recognised the error of this view, it is one which nevertheless still largely prevails, and can only be overcome by a persevering study of the patho logical changes undergone by the stomach, which underlie these disturbances in its functions. Backing up the pioneering medical researchers and experi menters are the phalanxes and cohorts of practising clinicians in district general hospitals and in general practice who may have to implement and apply any breakthroughs and advances in practical and realistic terms. This they cannot, and should not, be expected to do without careful consideration and analysis. It is essential, therefore, to have regular reviews of the growing points of medicine which are constructively critical as well as being enthusiastic and which can present the issues and implications clearly and fairly to clinicians. The Practical Clinical Medicine series is designed to provide such regular reviews on selected subjects. Each volume is under the charge of an invited editor who selects his team of 4--6 experts. Each contribution is an authoritative, detailed and referenced examination of his topic, is clearly presented in an understandable manner and is practical, relevant and applic able to everyday clinical practice. The series is intended as a means of communication between researchers and practising clinicians. It is dedicated to gener alists who provide primary health care in general practice and to generalists providing secondary medical care in district vii viii Series Editors' Foreword general hospitals. Both are involved in applying good general practical clinical medicine for their patients, but can only succeed in a climate of constant review and examination.

Dyspepsia in particular remains a complex problem in clinical practice once an organic cause is excluded. Many recent attempts to find new treatment strategies have failed: Tegaserod, for example, has been withdrawn from the United States, and itopride, which showed promising results in phase 2 trials, ultimately failed in phase 3 trials. Future treatments will depend to a significant extent on the careful evaluation of patients and an analysis of their symptoms, as well as on new insights into the pathophysiology of symptoms in the upper GI tract. This publication provides an overview of the current state of dyspepsia diagnosis and management, evaluating the pathophysiology, current diagnostic and treatment strategies, as well as the results of 'Helicobacter pylori' eradication and proton pump inhibitor therapy. Moreover, future options for therapy of this common disorder are assessed, making this compilation a most welcome reference work for the clinician.
