Outcome measures included Quadruple Visual Analogue Scale, Short Form McGill Pain Questionnaire, Headache Disability Index and headache diary. Osteopathic treatment involved high velocity low amplitude manipulation, muscle energy technique, strain-counterstrain, myofascial release and cranial osteopathic techniques. Visual analysis of plotted outcome measure data indicated a reduction in intensity, frequency and disability related to headache pain.

BackgroundMigraine pain has a multidimensional dimension affecting the central nervous system (CNS) primary and secondary somatosensory cortices, thalamus, insula, anterior cingulate cortex and parietal and prefrontal cortices.It is a chronic disease that reduces the quality of life (QoL) presenting nausea, vomiting, and/or photophobia. The origin of migraine is argued but there is evidence about a vegetative nervous system (VNS) especially in relation to the central sensitization for an alteration of the trigemino-vascular-nuclei and meningesu2019 inflammation (Fig. 1). Migraine occurs in about 15% of the general population with a total cost per year estimated of u20ac18 billion in Europe, more than neurological disorders (multiple sclerosis, Parkinsonu2019s disease and stroke or brain tumour). Migraine is usually managed by medication, but nonpharmacological management could be an alternative treatment option to reduce pain, disability, increase QoL and reduce costs.AimsThe aim is to identify and critically evaluate the available literature regarding the effectiveness of osteopathic treatment (OMT) in patients affected by migraine with regards to pain and disability.MethodsA systematic review has been conducted, screening and reviewing more than 10 among databases and journals to answer the aim.ResultsOMT may be considered a valid procedure for the management of migraineurs. All studies reflect a significant improvement in pain and disability level (p u2264 0.01) (Fig. 2).ConclusionsOMT through spinal manipulation technique or craniosacral therapy aims to influence the CNS altering somatosensory processing at the cortical level neuromuscular-autonomic regulatory mechanisms to reduce pain.Although debate about the cause of migraine is still open, osteopathy would allow to address the pathology with a non-drug and non-invasively therapy.References:Posadzki, P. and Ernst, E. 2011. M. WILKINSON Patients with frequent or daily headaches pose a very difficult problem for the physician who has to treat them, particularly as many patients think that there should be a medicine or medicines which give them instant relief. In the search for the compound which would meet this very natural desire, many drugs have been manufactured and the temptation for the physician is either to increase the dose of a drug which seems to be, at any rate, partially effective, or to add one or more drugs to those which the patient is already taking. Although there have been some references to the dangers of overdosage of drugs for migraine in the past, it was not until relatively recently that it was recognized that drugs given for the relief of headache, if taken injudiciously, may themselves cause headache. The first drugs to be implicated in this way were ergotamine and phenazone. In the case of ergotamine tartrate, the dangers of ergotism were well known as this was a disorder which had been known and written about for many years. In the treatment of headache, fully blown ergotism is rare and in recent years has usually been due to self-medication in doses much greater than those prescribed although there are a few recorded cases where toxic amounts have been given.
