Continuity of Care Record (CCR) is a health record standard specification developed jointly by ASTM International, the Massachusetts Medical Society (MMS), the Healthcare Information and Management Systems Society (HIMSS), the American Academy of Family Physicians (AAFP), the American Academy of Pediatrics (AAP), and other health informatics vendors. The Continuity of Care Document (CCD) specification is an XML-based markup standard intended to specify the encoding, structure and semantics of a patient summary clinical document for exchange. Continuity of Care Document (CCD) r1 is an HL7 CDA implementation of the Continuity of Care Record (CCR) and not a competing standard as mentioned by some. It is generally possible to convert a CCR document into CCD using Extensible Stylesheet Language Transformations (XSLT), however it is not always possible to perform the inverse transformation as some CCD features are not supported in CCR. Electronic health records (EHRs) are considered an essential ingredient of care coordination by Medical Homes; however, detailed descriptions of how it actually happens in real life are rarely found. eHealth Initiative, working with sanofi-aventis and Health & Technology Vector (H & TV), recently concluded an exploratory project to understand how eHRs can be used to improve care coordination for complex patients. The project provided a multi-dimensional picture of this one element of transforming primary care to the medical home. It began with an operational definition of, and measures for, care coordination. Then, the Care Coordination project built and demonstrated two models for improving care coordination between medical homes and specialists.
