This report is the summary of the fourth workshop of The Roundtable on Environmental Health Sciences, Research, and Medicine. Environmental Indicators: Bridging the Chasm Between Public Health and the Environment, continues the overarching themes of previous workshops on rebuilding the unity of health and the environment. The purpose of the workshop was to bring people together from many fields, including federal, state, local, and private partners in environmental health, to examine potential leading indicators of environmental health, to discuss the proposed national health tracking effort, to look into monitoring systems of other nations, and to foster a dialogue on the steps for establishing a nationwide environmental health monitoring system. We use the classical model of structured expert judgment to examine the return on investment (ROI) in a health surveillance network. The US Centers for Disease Control and Prevention launched the Environmental Public Health Tracking Program (Tracking Program) in 2002 to collect available data to better understand the link between health and the environment. The Tracking Program's annual budget is $25 million, but little is known about the value of its potential benefits. Past evaluations of the Tracking Program have focused on its ability to meet its own goals rather than on broader impact. This study is the first quantitative assessment of the ROI of the Tracking Program. We make novel use of structured expert judgment to establish a counterfactual allowing us to examine the program's impact. We then link the estimated impact to existing literature on the economic benefits associated with the improved indicators. We estimate that the Tracking Program may have yielded between $5.22 billion to $17.1 billion in societal benefits in 2011, with $6.24 billion estimated by the optimized combination of experts. The estimates have a wide range of uncertainty, but these amounts are significantly greater than the annual $25 million cost of the program.

"In January 2001, the Pew Environmental Health Commission called for the creation of a coordinated public health system to prevent disease in the United States by tracking and combating environmental health threats. In response, the United States Congress appropriated funding to the Centers for Disease Control and Prevention (CDC) in Fiscal Year (FY) 2002. This funding enabled the development of CDC's National Environmental Public Health Tracking (EPHT) Program, which is led by the National Center for Environmental Health's (NCEH) Environmental Health Tracking Branch (EHTB). The purpose of the EPHT Program (the Program) is to provide information from a nationwide network of integrated health and environmental data that drives actions to improve the health of communities. This National Network (the Network) will integrate three distinct components: hazard monitoring, exposure surveillance, and health effects surveillance. "In September 2000, after 18 months of careful study and review, the Pew Environmental Health Commission released a report on the state of environmental public health in the United States. The Commission reported that the environmental public health system in the United States was fragmented, neglected, and ineffective. The first recommendation by the Commission was for the federal government to establish a national EPHT network to link information on environmentally related diseases, human exposures, and environmental hazards. The turn of the millennium has been accompanied by a rapid growth in data collection along with an increasing ability to store, manipulate and analyze it. In tandem with this development in technology and surveillance, there has been a growing understanding of the importance of the socio-physical environment on population behavior and human health. We capitalize on this progress to develop a methodology by which we develop two macro scale datasets, one national and one state-wide, to support the efforts of the Centers for Disease Control and Prevention's Environmental Public Health Tracking Network (EPHTN) in understanding two important health risks: smoking and obesity. Moreover, we use new geographic information science techniques, spatial statistics methodologies and machine learning algorithms to gain a better understanding of the relationship between spatial patterns in physical and socio-demographic characteristics and health risks. Specifically, we address three specific aims: (1) To use current data systems to develop national small-area predictions of adult smoking and obesity for the EPHTN and research; (2) To evaluate current data systems and spatial analysis tools available to describe the within-school environment and the school-neighborhood socio-physical characteristics of California's public schools thought to influence childhood obesity, and use these to develop a comprehensive multilevel dataset for the EPHTN and research; (3) To test the utility of the dataset developed in aim 2 by applying it to an analysis used to increase the understanding of the relationship between the school environment and childhood obesity by examining the relative importance of school attributes. This analysis determines that current data collection systems provide a valuable resource which we combine for the ease of future research use. We demonstrate that using the spatial structure and socio-demographic patterns of health risks, we are able to downscale adult smoking and obesity prevalence to the ZIP code and census tract levels for the conterminous United States, and develop five-year predictions for these for the four quinquennia in the 1991-2010 time period. Lastly, we confirm the utility of the school dataset and determine through the third aim that individual demographics and the social environment seem to be the predominant determinants of childhood obesity.
