Patient mobility across Europe is markedly increasing and new generations will actively ask to be treated by the health-care system that best meets their needs. At a political level, the EU issued the EU Directive no. 24/2011/CE of 9th March 2011 concerning the application of patients’ rights in cross-border health care and has contributed to improving the level of freedom of choice for the European citizen, but it does not seem to have increased actual patient mobility across Europe. Freedom to choose is necessary to grant the people of Europe the same access to public-sector health-care services. The latter is a key instrument for an efficiently functioning “single market” ensuring real mobility within the EU. The aim of this book is to study the current European health care market and discuss the hypothesis of a European right of citizenship with reference to health-care services. It examines patients' mobility from several perspectives: determinants of patient mobility, governance of cross-border mobility at EU level as concerns patients and health-care professionals, policy implications, and case studies. It is intended for health researchers, decision-makers and professionals concerned with health-care provision and patient mobility. The goal is to provide, through scientific and methodological rigor, new informative tools useful for the implementation of new policies in the health-care sector in order to implement effective health-care integration in the European Union.

In particular, this report looks at the inequality in access to healthcare and long-term care between men and women in the EU. Healthcare regulations should guarantee that everybody has access to appropriate healthcare. The main goals for healthcare are: Equal access to health care for everyone; Cost-efficient production of health services and Cost-control of public expenditure for medical services. Especially cost-control seems to be a global problem. One of the key issues in the debate on how to improve healthcare is rationing. It is an important challenge to understand the various methods of rationing in medical care, to analyse the effects of rationing and the ways to harmonize the various rationing cultures in Europe. This publication gives a comprehensive overview of the perception of different population groups in an international context and it shows how the different population and occupational groups estimate the possibilities, forms and also limits rationing in the near future. Patients who are affected by rationing decisions could ask for treatment in foreign European countries. In order to limit social inequities caused by capacity problems in less rationed countries it is important in a first step to demonstrate the differences in rationing procedures between European countries.

Demographic, epidemiological, social and cultural trends in European countries are changing the traditional patterns of care. The next decades will see rising numbers of care-dependent older people and noncommunicable diseases as the leading cause of chronic illness and disability. Further, urbanization and the break-up of the traditional large family group will lead to gaps in the care of older or disabled family members. These changes in needs and social structure require a different approach to policy and services in the health and social sectors; a disease-oriented approach alone is no longer appropriate. Home care could be an answer to these issues: a sustainable approach to prevent the need for unnecessary acute or long-term institutionalization and to maintain people in their homes and communities as long as possible. Along with new forms of service delivery organization, technological innovation can represent a viable solution to developing home care in Europe, provided that health care systems can further enhance integration and coordination. This publication is part of the work of the WHO Regional Office for Europe to present evidence for health policy- and decision-makers in a clear and understandable form. It explains why health and social services should provide high-quality and targeted home care for disabled and older people. It provides evidence for the effectiveness of home care, shows how it can be improved, and explains the need to ensure equitable access. The publication also explores the varied cultural and care contexts in different countries, and reveals how to educate professionals and the public about these issues. This booklet seeks to broaden awareness, stimulate debate and promote action.
