Within the field of general medicine, the last two decades have seen the curative procedures increasingly supplemented by func tions that can be summed up under the heading of "preventive health care." There is a close link between this development and the changing morbidity pattern. In highly developed countries the morbidity rate attributable to infectious diseases has been much reduced, while the various types of arteriosclerosis, some tumorous conditions, and so-called functional syndromes have attained massive incidence rates. Systematic epidemiologic studies have yielded the concept of risk factors, i. e., certain exogenous and endogenous factors whose elimination, ideally, would make it possible to avoid the manifestation of an illness or to ensure it did not become mani fest until later in life or in a milder form. The socioeconomic and sociomedical significance of these aspects is considerable, as can be seen from all the statistics on the reasons for incapacity and early retirement and on the relative contributions of different ill nesses to total mortality. Effective avoidance of illnesses by primary prophylaxis (elimi nation of the causes of disease) or early detection of the symp toms of disease (secondary prophylaxis) leading to early treat ment would be a particularly significant step forward, since in the case of most of the conditions mentioned above only pallia tive forms of treatment are available, and none that have any cu rative effect.

The measurement oflactate is straightforward and reliable. The sampling is normal ly dependable, although discrepancies between sample sites can occur if samples are drawn during other than steady state conditions or when blood flow to the site is low. Comparison of data between subjects should consider intra- and interindividu al variation. Interpretation of the significance of blood or even a muscle lactate concentration is extremely difficult and neither can be assumed to reflect rates of lactate release or production. Lactate metabolism appears to be very significant in short-term, high intensity work lasting a few seconds, but the exact role remains to be established. Blood lactate and the pulmonary aspects of anaerobic threshold are not necessarily causally related and the significance of anaerobic threshold as a metabolic measure remains obscure. There are numerous examples of changes in blood or muscle lac tate independent of oxygen availability, and factors such as H+ concentration ap pear to play an important, although undefined, role in regulating lactate production and/or release.

Exercise testing is widely used all over the world to assess functional capacity in athletes, healthy subjects and patients. According to recent surveys, the interest in ergometry is still growing in almost all fields of medicine, especially in private practice. Furthermore, there has been an exponential growth in the number of publications on exercise testing in the last years. Several consensus and task force conferences have dealt with exercise testing and published recommendations on standardization and guidelines in ergometry. These factors have, in combination, initiated an upsurge in research and clinical use of exercise testing. At the 6th International Seminar on Ergometry the latest findings and advances in ergometry were discussed. Reviews and results of the congress covering a wide range of features in exercise testing are presented in this book. The editors hope that this book will make a substantial contribution to our knowledge regarding exercise testing and will help physicians to appropriately evaluate exercise testing in healthy and diseased subjects. Physical Work and Effort is a collection of papers presented at the Proceedings of the First International Symposium held at Stockholm on December 2-4, 1975. This book deals with the investigations done on the clinical physiology of work and effort. Part 1 discusses basic theories; methods and results that can be found in studies relating to fatigue; visual perception; and stress that includes a simple method of rating these estimates of perceptions. Part 3 covers topics on clinical and applied studies such as ECG changes in asymptomatic men; perceived pain during tread-mill exercise; and changes in heart rate during work exertion or when driving under traffic conditions. Part 4 evaluates the psychophysiological intercorrelations from a series of experiments and concludes that two factors are present in the perception of exertion during physical work—the local factor and a central factor. The book also includes the use of a model to evaluate the perceived ratings of these two factors. Psychologists, physiologists, physical therapists, physiotherapists, and scientists involved in work improvement will find this book very valuable.
