Verocytotxin producing Escherichia coli (VTEC), and in particular, strains of serogroup o157, have emerged as significant pathogens causing a range of severe and potentially fatal illnesses. The European Union has recognised the threat posed by E. coli O157:H7 and the need to devise control strategies based on an understanding of VTEC pathogenicity, transmission, survival and growth. It also acknowledges the imortance of informing farmers, veterinarians, food producers and health authorities so that each of these groups can act appropriately to reduce the overall hazards posed by these organisms. To contribute to the development and dissemination of effect control strategies, the European Commission funded a Concerted Action Project "A European study on animal, food, and biomedical aspects of verocytotoxigenic E. coli including serotype O157:H7, an emerging pathogen" (CT98-3935) within the Agriculture and Agro-industry Framework IV Research Programme (1998-2001). This book, compiled under the auspices of the above project, integrates contributions from project participants and invited contributor, to provide a comprehensive overview of the current state of research on VTEC. E. coli O157:H7, since its discovery as a human foodborne pathogen in 1982, has been recognized as a disease agent causing hemorrhagic colitis, hemolytic uremic syndrome and thrombotic thrombocytopenia purpura. Since E. coli O157:H7 is a more environmentally sensitive organism than other E. coli strains, and since foods containing low numbers of cells have caused illness, there have been difficulties in isolating and identifying the organism from food. The proceedings of this workshop document the present methods to detect E. coli O157:H7 and other verotoxigenic organisms in food. Data gathered from surveys of animals at the slaughterhouse, retail meats and ready-to-eat foods in the US and Canada are also presented. In addition, plans for further methodology research, including collaborative studies; and recommendations concerning the best procedures for detection of these pathogens are outlined.

USDA's Food Safety and Inspection Service (FSIS) is formulating risk assessments to identify important foodborne hazards; evaluate potential strategies to prevent, reduce, or eliminate those hazards; assess the effects of different mitigation strategies; and identify research needs. These risk assessments, in brief, empirically characterize the determinants of the presence or level of microbial contamination in vulnerable foodstuffs at various points leading up to consumption. One of the initial efforts in the undertaking is a risk assessment of the public health impact of E. coli O157:H7 in ground beef. In addition to soliciting public input, FSIS asked the Institute of Medicine (IOM) to convene a committee of experts to review the draft and offer recommendations and suggestions for consideration as the agency finalizes the document. This report presents the results of that review.

The most notorious type of pathogenic E. coli is known as E. coli 0157: H7. Over a quarter century after the discovery of verocytotoxin and the first report by Karmali and colleagues of cases of postdiarrheal hemolytic uremic syndrome (HUS) caused by verotoxigenic Escherichia coli (VTEC), otherwise known as "Shiga toxin-producing" E. coli, or STEC for short, successful treatment of these infections has remained elusive. When a child is lost to something as preventable as E. coli, it is appalling. Outbreaks of E. coli 0157: H7-induced illness have been common in recent years. With all of the regulations and inspections, one would think our food supply would be free from E. coli contamination. That is not true. Outbreaks and recalls happen frequently. In 2011, a deadly outbreak began in Europe due to a rare strain of E coli, Shiga toxin-producing E. coli O104, that produces a serious illness similar to that produced by E coli 0157: H7.
