La esquizofrenia es uno de los trastornos mentales más graves y enigmáticos, con alteraciones que afectan a todas las funciones y ámbitos psicológicos, incluyendo cambios en la percepción, la cognición, el pensamiento y lenguaje, y el sistema motor, lo que se refleja en un amplio y complejo cuadro de síntomas y comportamientos peculiares (alucinaciones, ideas delirantes, incoherencia, discurso desorganizado e incomprensible, catatonia, mutismo, estupor, conducta extravagante, inexpresión emocional, incongruencia en la expresión afectiva, etc.). Por esta razón la naturaleza de la esquizofrenia es muy compleja e implica a múltiples factores, tanto de tipo genético como ambiental, tanto de tipo neurobiológico como psicológico. Aunque los factores psicológicos pueden jugar un papel importante, especialmente el estrés durante etapas críticas del desarrollo, la investigación reciente ha puesto de relieve importantes avances en el campo de la genética (descubrimiento de nuevos genes implicados), la neurofisiología (anomalías en determinas áreas cerebrales) y la bioquímica (alteraciones en determinados mecanismos de neurotransmisión del sistema nervioso central). En el libro se actualizan estas líneas de evidencia, y se propone un modelo de neurodesarrollo que integra los factores neurobiológicos y los factores ambientales. El modelo describe aspectos cruciales para el desarrollo de la esquizofrenia como la epigenética (puerta de acceso de la influencia del ambiente sobre los genes), la poda sináptica (refinamiento sináptico), y la maduración de las trayectorias GABAérgicas y dopaminérgicas, destacándose el papel de las distintas etapas críticas en los procesos de neurodesarrollo. Finalmente se comentan las directrices actuales y futuras sobre la evaluación, el tratamiento (farmacológico y psicológico) y la prevención.
