Until recently, it was thought that the adult brain is modifiable only during early stages of ontogenesis. However, neurophysiological and neuroimaging studies now indicate that the mature human brain is, under certain conditions, capable of substantial neuroplastic changes. Neuroplasticity reflects the ability of the human brain to alter the pattern of neural activation in response to previous experience, and recent findings indicate that the effects of experience can lead to both structural as well as functional reorganization. It has been shown that pathological neuroplastic changes can be reverted/normalized and that the modulation of the neuroplastic changes can be paralleled by improvement of the patient's status. However, there is a gap between the potential of neuromodulation, technical progress and actual preparedness of medical personnel to provide this type of treatment. A prevalent opinion among medical professionals indicates that training programs and educational materials in neuromodulatory techniques are well needed and appreciated. Neuromodulation will focus on the description and discussion of methods currently available for invasive and non-invasive neuromodulation, their clinical potential, significance and practical applications. In order to facilitate understanding of the topic, the initial part of the textbook will review neurophysiological systems involved in neuromodulation and will provide readers with basic principles of neuroplasticity that constitutes the rationale for neuromodulation in human medicine. Additionally, the clinical use of these techniques will be described with special regard to safety and avoidance of complications.

Could neurostimulation be a management option for your patients? Neurostimulation techniques present real management options for patients with a range of neurologic and psychiatric disorders, such as movement disorders, pain, and depression. They should be actively considered when conventional medical approaches have failed or are inappropriate. But for many clinicians, these new methods pose many questions. What are the available modalities? How do they work? Which patients might benefit from them? How do I explain the processes to patients? How do I monitor my patient’s progress after implantation? Neurostimulation: Principles and Practice provides a concise, easy-to-read fusion of the clinical applications of implanted neurostimulators. It demystifies selection and referral criteria, maximizing therapy, programming the implanted neuromodulators, monitoring progress, and troubleshooting problems associated with neurostimulation. Neurostimulation: Principles and Practice covers the modalities available for your patients: Deep brain stimulation Motor cortex stimulation Vagus nerve stimulation Spinal cord stimulation Peripheral nerve stimulation Written by an international cast of experts, Neurostimulation: Principles and Practice sets the stage for you to provide real clinical benefit to your patients who might receive, or are already using, neurostimulators.

Developed over the past 25 years, Cortical Brain Stimulation has emerged as a brand new, cutting-edge option for the treatment of intractable neurological and psychiatric disorders. Devoid of the mortality and disabling morbidity that may accompany deep brain stimulation, stimulating the cortex with a minimally invasive surgical approach had initially proved its worth for the treatment of Central and other Neuropathic Pain Syndromes and later for Parkinson Disease, Dystonia, Stroke and Coma rehabilitation, Epilepsy, Depression and Tinnitus. Written by many of the pioneers in the field, this authoritative treatise is a comprehensive presentation - from surgical details, to clinical results and mechanisms of action. It also provides the busy clinician with comparisons with non-invasive cortical stimulation techniques, such as TMS and tDCS. No other book deals with this form of brain stimulation. The clinician will harness the power of this formidable new therapeutic option, which is being further refined with the advent of closed-loop stimulation. Dr Canavero deciphered the genesis of the central pain syndromes, introduced extradural cortical stimulation for Parkinson Disease and the vegetative state and co-introduced extradural cortical stimulation for stroke rehabilitation. He made worldwide news in 2008 for partially restoring consciousness in two vegetative patients, in 2013 for proposing the HEAVEN/GEMINI protocol for human head transplantation and in 2014 for pushing brain stimulation in the setting of criminal psychopathy. His books include: Central Pain Syndrome, Cambridge Univ. Press, 2011 (2nd ed.), Textbook of therapeutic cortical stimulation, Nova Sci, 2009 and two books in Italian on human sexual behavior.
