El objetivo principal de la presente tesis fue estudiar la fenomenología y la dinámica de posibles mecanismos psicológicos relacionados con el inicio de la psicosis. Para ello se utilizó tanto una muestra de personas en riesgo psicométrico de desarrollar una psicosis, como una muestra clínica compuesta por personas con estado mental de alto riesgo para la psicosis (EMAR), y personas que habían sufrido un primer episodio psicótico (PEP). En cuatro de las seis investigaciones empíricas presentadas se utilizado la metodología de evaluación ecológica momentánea (EMA), la cual mediante la evaluación repetida en tiempo y en el contexto real de ocurrencia de los distintos constructos evaluados, ofrece una información complementaria y de mayor validez ecológica que la ofrecida por la evaluación clínica y psicométrica tradicional.Los resultados de esta tesis muestran que en una muestra no clínica la interacción entre los rasgos paranoides y depresivos conlleva una auto-representación consciente más negativa, y una elevada discrepancia entre la autoestima implícita y explícita, que podría significar un riesgo aumentado hacia la psicopatología. En la muestra clínica se encontró que la autoestima explícita, rasgo y estado, se asoció con la ideación paranoide en la vida diaria, mientras que no se encontraron asociaciones entre la autoestima implícita y la paranoia ni en el grupo EMAR ni en el grupo PEP. La ansiedad y el apoyo social modularon las relaciones entre la autoestima y la paranoia, siendo el sentimiento de sentirse apoyado por los demás un factor de protección clave respecto al posible impacto negativo que la baja autoestima podría ejercer sobre la ideación paranoide en la vida diaria. En el siguiente estudio se encontró que la tristeza, la ansiedad, y la baja autoestima, mediaron en parte la asociación entre el estrés y los síntomas positivos, mientras que la tristeza y la baja autoestima, pero no la ansiedad, mediaron la relación entre el estrés y la paranoia. De la misma manera, la asociación entre la autoestima y los síntomas psicóticos (y la paranoia) estuvo mediada por la ansiedad y la tristeza, aunque la tristeza resultó tener una contribución relativa mayor. También, se analizaron las relaciones causales entre distintos factores psicológicos y los síntomas positivos, encontrando que el estrés, tristeza, y el apoyo social percibido, predijeron los subsiguientes niveles de paranoia y síntomas positivos en la vida diaria. La ansiedad se relacionó con un subsiguiente incremento de los síntomas positivos, pero no de la paranoia, mientras que estar en un entorno social no deseado incrementó la paranoia, pero no los síntomas positivos. Los resultados que analizaron el impacto del ambiente familiar en la expresión de los síntomas positivos mostraron que la asociación entre la emoción expresada de los familiares y los síntomas positivos de los pacientes estuvo mediada de manera serial primero, por la emoción expresada percibida por los pacientes, y seguidamente por los niveles de autoestima negativa de los propios pacientes. Además, los niveles de malestar emocional de los pacientes modularon esta asociación. Finalmente, se realizó un estudio psicométrico del principal instrumento utilizado para medir la depresión en la esquizofrenia, donde se demostró su validez y fiabilidad en una muestra compuesta por personas en riego psicométrico de desarrollar psicosis, pacientes con EMAR, y pacientes con PEP.En general, los resultados de esta tesis subrayan la importancia de los procesos afectivos, y de la autoestima, en la generación y mantenimiento de los síntomas psicóticos positivos, tanto a nivel directo como en interacción con otros factores como el estrés o el ambiente familiar. Además, demuestran la necesidad de refinar y ampliar la comprensión de las complejas interacciones psicosociales que subyacen en la génesis de la psicosis.
