This volume inaugurates a series concerning philosophy and medicine. There are few, if any, areas of social concern so pervasive as medicine and yet as underexamined by philosophy. But the claim to precedence of the Proceedings of the First Trans-Disciplinary Symposium on Philos ophy and Medicine must be qualified. Claims to be "first" are notorious in the history of scientific as well as humanistic investigation and the claim that the First Trans-Disciplinary Symposium on Philosophy and Medicine has no precedent is not meant to be put in bald form. The editors clearly do not maintain that philosophers and physicians have not heretofore discussed matters of mutual concern, nor that individual philosophers and physicians have never taken up problems and concepts in medicine which are themselves at the boundary or interface of these two disciplines - concepts like "matter," "disease," "psyche. " Surely there have been books published on the logic and philosophy of medi 1 cine. But the formalization of issues and concepts in medicine has not received, at least in this century, sustained interest by professional phi losophers. Groups of philosophers have not engaged medicine in order to explicate its philosophical presuppositions and to sort out the various concepts which appear in medicine. The scope of such an effort takes the philosopher beyond problems and issues which today are subsumed under the rubric "medical ethics.

This is the first monograph to deal with medicine as a form of hermeneutics, now in a thoroughly revised and updated edition, including a whole new chapter on medical ethics. The book offers a comprehensive philosophical argument why good medical practice cannot be curtailed to scientific investigations of the body but is a form of clinical hermeneutics performed by health-care professionals in dialogue with their patients. Medical hermeneutics is rooted in a phenomenology of illness which acknowledges and proceeds from the ill party’s bodily feelings, everyday life-world circumstances and self-understanding in aiming to restore health. The author shows how the works of classical phenomenologists and hermeneuticians – Martin Heidegger, Maurice Merleau-Ponty, Hans-Georg Gadamer and Paul Ricoeur – may be employed to understand how medical diagnosis is enveloped by professional empathy and clinical judgement and developed by scientific investigations of the patient’s bodily condition. Health and illness are ultimately considered to be ways of feeling at home or not at home in the world, and such experiences are the starting point of medical hermeneutics when aiming to make best use of scientific knowledge. The germs of the ideas in this book became implanted in me during my experience as a resident in clinical pathology at Boston University Medical Center. At the time, I had inklings that the test results churned out by our laboratories were more than scientific facts. As a philosophically unsophisticated young physician, however, I had no language or framework to analyze what I saw as a deep philosophical problem, a problem largely unrecognized by most physicians. The test results provided by our laboratories were accurate and of great practical importance for patient care. However, most of the physicians who relied on our test results to diagnose and treat their patients either did not have the time or interest to consider the philosophical issues inherent in diagnosis, or, like me, had inadequate means to further analyze them. It was more than ten years later that I began doctoral studies in philosophy, and I was fortunate to find a faculty that was supportive of my efforts to address the problem. This book began as my doctoral dissertation in the Department of Philosophy at Georgetown University. I would like to acknowledge the assistance of my mentor, Robert Veatch, Ph. D. There has been much recent excitement amongst neuroscientists and ethicists about the possibility of using drugs, as well as other technologies, to enhance cognition in healthy individuals. This excitement has arisen from recent advances in neuroscientific technologies such as drugs that increase alertness and wakefulness in healthy individuals or technologies that can stimulate activity in different parts of the brain - either via the scalp or via electrodes - raising the possibility of producing cognitive and affective improvements in otherwise healthy individuals. Despite this growing interest, there are conflicting views on the ethics of cognitive enhancement. Some argue that enhancement is not only an ethical pursuit but one that we have a moral obligation to pursue. Others are more skeptical about the ethical implications and long term effects of cognitive enhancement.
