Evidence-based guidelines for discharge instructions, when successfully administered, reduce readmission rates in patients, leading to improved quality of life and economic savings. Unfortunately, effective delivery is complex and time consuming, placing a high demand on already overworked bedside nurses. Failure to provide complete discharge instructions can result in non-adherence to patient treatment regimens and lack of essential follow-up, the most commonly identified reasons for readmissions. To improve quality of care, hospitals need to adopt a new model that incorporates delivery of intensive, one on one education to patients during the hospital stay with continuing support, guidance, and education throughout the transition from hospital to home. The purpose of this study was to determine the relationship between the education provided by a staff registered nurse and a clinical patient educator, a registered nurse who specializes in specific disease processes. An exploratory descriptive study design was used to examine and describe the perception of registered nurses regarding discharge education provided by a clinical patient educator who specializes in disease processes versus that provided by a staff registered nurse. An Independent samples t test was conducted comparing the mean of each survey question for those working in acute care to those not working in acute care. Only two survey questions' mean answers were statistically significantly different between the two groups. Those nurses not working in acute care felt more strongly that they spent, on average, more than 30 minutes with discharge teaching than those working in acute care. Those nurses working in acute care felt more strongly that they spent less than 30 minutes with discharge teaching than those nurses not working in acute care.

Based on recent studies, patient education has become an important topic that has risen within society as a way to increase patient self management within patient care. More patients have left ignorant to their condition and have no idea of self management after discharge, which leads to frequent visits from the same patients. With the study, it analyzes the benefits of patient education through different conditions. In addition, the study also recognizes how important it is for nurses to expand their knowledge within the practice. Throughout the study, the idea of patient education is a rising item within healthcare. In addition, different theories have been recognized, but according to Dorothea Orem's theory, she believed that patients must participate in the practice of activities that the individuals initiates and perform on their own behalf in maintaining life, health and well being. Therefore, the purpose of the patient education is to help patients fall in the habit of taking initiative in self management and also help nurses take advantage of learning new ways of educating patients about the importance of their conditions to prevent errors within healthcare, which seems to be a major problem. Patients and family members should be educated about their condition and ways to initiate care after being discharged. In the project, a new policy will be created in order to initiate change within each unit of the healthcare system. The nurse leaders will call meetings that will explain the purpose of patient education in depth through various levels of teaching tools that will allow each nurse to personally explain how they feel about the idea presented before them. In addition, the nurse leaders in charge, after accepting a unanimous agreement to continue on, will later on present the idea of directors of nursing who will have a lasting effect on how the hospital will accept the change. A study was introduced about patients who were paralyzed and their benefits of self care through patient education. The study accessed ten patients and educated 5 out of 10 of them. The Running head: Constructing the Written Evidence-Based Proposal 3 five that were thoroughly educated were able to take care of themselves after being discharged, while those who were not educated completely were left in the dark about ways to help and improve their condition.
