Research has shown the importance of monitoring behavior patterns of older adults over extended time periods to detect and deviations. These deviations can help determine health changes which could be prevented by early intervention. However, detecting these behavior patterns can be difficult using wearable sensors. These require the user to continuously wear the device which may not be acceptable for older adults. To address this, we propose an unobtrusive method to detect and measure the behavior patterns of older adults in their apartments using depth sensors. The many advantages of this approach include the low cost, unobtrusive, great performance in low illumination. An initial study, conducted in laboratory settings, detects activities in controlled settings. This study was further validated from data collected at TigerPlace, an independent facility for older adults in Columbia, MO. Sedentary behavior patterns of older adults with different physiologies and lifestyles are explored and results are reported. Activities of daily living (ADLs) are explored and a framework for recognizing ADLs in unstructured as well as unknown settings without prior information are explored. This can be coupled with activity information obtained from other sensors such as bed sensors, acoustic sensors, and motion sensors to generate more descriptive activity patterns. An in-home activity monitoring system would benefit greatly from our algorithm to alert healthcare providers of significant temporal changes in ADL behavior patterns of frail older adults for fall risk and cognitive impairment.

INTRODUCTION: Little research to date has explored typologies of physical activity among older adults. An understanding of physical activity patterns may help to both determine the health benefits of different types of activity and target interventions to increase activity levels in older adults. This analysis, conducted in 2014, used a latent class analysis approach to characterize patterns of physical activity in a cohort of older adults. METHODS: A total of 3,497 men and women aged 65-75 years living in New York City completed the Physical Activity Scale for the Elderly (PASE) in 2011. PASE scale items were used to classify subjects into latent classes. Multinomial regression was then used to relate individual and neighborhood characteristics to class membership. RESULTS: Five latent classes were identified: "least active," "walkers," "domestic/gardening," "athletic," and "domestic/gardening athletic." Individual-level predictors, including more education, higher income, and better self-reported health, were associated with membership in the more-active classes, particularly the athletic classes. Residential characteristics, including living in single-family housing and living in the lower-density boroughs of New York City, were predictive of membership in one of the domestic/gardening classes. Class membership was associated with BMI even after controlling for total PASE score. CONCLUSIONS: This study suggests that individual and neighborhood characteristics are associated with distinct physical activity patterns in a group of older urban adults. These patterns are associated with body habitus independent of overall activity.

The public health implications of growing numbers of older adults at risk for dementia place pressure on identifying dementia at its earliest stages so as to develop proactive management plans. However, this capability of early detection is very challenging with the contemporary detection process in the form of conventional clinician visits, which are typically not sensitive to detecting mild cognitive or functional decline. Fortunately, some studies have documented that changes in motor capabilities precede and may be indicative of cognitive impairment. A major motivation for this thesis was: can we utilize changes in motor capabilities and activity patterns to automatically predict the onset of dementia in older adults through continuous monitoring using unobtrusive sensors? Many studies have demonstrated that the prodromal dementia phase commonly identified as mild cognitive impairment is an important target for this early detection of impending dementia amenable to treatment. Consequently, in this thesis we explored different approaches for the automatic detection of mild cognitive impairment using unobtrusive sensing technologies. We started off by exploring the feasibility of detecting mild cognitive impairment in older adults using a number of predefined measures associated with their in-home walking speed. We were able to achieve this goal with an area under the ROC curve and an area under the precision-recall curve of 0.97 and 0.93, respectively, using a time frame of 24 weeks. We were also very interested in exploring the feasibility of detecting mild cognitive impairment in older adults using changes in their activity patterns. We used inhomogeneous Poisson processes to build generalized linear models of older adults' activity that would model their presence in different rooms throughout the day. Using these models, we extracted very interesting insights and visualized significant changes in older adults' activity patterns as they started experiencing cognitive impairment. Finally, we devised a method for automatic detection of mild cognitive impairment using changes in older adults' activity models, and we achieved this goal with an F0.5 score of 85.6 percent. We also investigated the two subtypes of mild cognitive impairment, namely the amnestic and the non-amnestic subtypes, and made very interesting observations.

The primary purpose of the present study was to identify the optimum variables to express the interrelationship between physical capacity (PC) and physical activity (PA) of community-dwelling older adults (65+), during activities of daily living, as measured by wireless health sensors. Second, it aimed to determine if PC was a predictor of change in a goal-setting intervention. Seventy-six (N = 76) older men and women (M age= 74.05 ± 5.15 years) had their PC measured in the laboratory using the MoveTest and then had their daily PA measured at home for one week, using the MoveMonitor. Twenty-three (n = 23) of these participants (M age = 73.13 ± 5.41 years) then set goals for step count, active time, and sitting time, and completed two weeks of monitoring/feedback. Correlation matrices and clustering identified ten variables to represent PC and PA. Canonical correlation analysis indicated a non-significant relationship between PC and PA.
