While the skeletal effects of vitamin D are well-documented, the role and importance of vitamin D outside of bone health has not been well-established. Vitamin D receptors are located in nearly every tissue of the body, and low levels of vitamin D are associated with a range of various diseases. This book provides an in-depth examination of these extraskeletal effects of vitamin D and the associations between vitamin D deficiency and various disease states. Beginning with a review of the biochemistry and physiology of vitamin D, subsequent chapters investigate its relationship to autoimmune and infectious diseases, various forms of cancer, endocrine issues such as diabetes, obesity and reproductive function, cardiovascular disease and muscle weakness. Concluding chapters discuss the role of vitamin D in neurological disorders, including Alzheimer's Disease, and cognitive function. Focusing on extraskeletal effects only across a range of conditions, Extraskeletal Effects of Vitamin D will be an important resource for clinical endocrinologists and primary care physicians.

There has been a progression of investigations of the biology and pharmacology of vitamin D. Original work proved the importance of this axis on bone and skeletal homeostasis. Subsequent knowledge of basic cellular physiology led to studies of the role of vitamin D in other tissues where calcium flux is important to cellular functions. This was the beginning of exploration of vitamin D in extraskeletal health. Next came investigations into extraskeletal diseases which appeared to be more prevalent in vitamin D-deficient individuals. It was surprising that not only was there higher mortality from those diseases associated with low vitamin D levels, but also all-cause mortality was higher as well. Cellular pathophysiology of these findings was explored. Finally, studies have focused on outcomes in seriously ill patients with those diseases when hospitalized in the intensive care unit (ICU). Inverse correlations have been seen of several common ICU outcomes with levels of vitamin D at entry to the ICU, but the current effort is now in clarifying a role for routine measurement of these levels and the possible role of at least vitamin D replacement or even supplementation in the ICU patient with multiple organ pharmacologic or mechanical life support.

Vitamin D deficiency is a worldwide problem linked to numerous diseases affecting men, women, and children of all ages. Enormous progress in the study of vitamin D has been made since the first edition of this highly-acclaimed book was published nearly 20 years ago, and current research continues to draw headlines. Feldman and Pike’s Vitamin D, Fifth Edition continues to build on the successful formula from previous editions, taking the reader from the basic elements of fundamental research to the most sophisticated concepts in therapeutics. The two comprehensive volumes provide investigators, clinicians, and students with a comprehensive, definitive, and up-to-date compendium of the diverse scientific and clinical aspects of vitamin D, where each area is covered by both basic and clinical experts in the field. In Volume I: Biochemistry, Physiology and Diagnostics, international experts in endocrinology, bone biology, and human physiology take readers through the basic research of vitamin D. This impressive reference presents a comprehensive review of the multi-faceted actions of vitamin D relating both to skeletal and extra-skeletal action. Researchers from all areas of vitamin D will gain insight into how clinical observations and practices can feed back into the research cycle and will, therefore, be able to develop more targeted genomic and proteomic insights into the mechanisms of disease. Volume II: Health, Disease and Therapy authoritatively covers the evidence for new roles of vitamin D, ranging from organ transplantation to cancer, diabetes, inflammatory bowel disease, multiple sclerosis, and renal disease. The coverage is appropriately broad, drawing on aspects of internal medicine, pediatrics, nutrition, orthopedics, oncology, neurology, obstetrics and gynecology, and immunology, as well as, new areas for vitamin D including sports medicine, opthalmology, veterinary medicine and ICU care – including COVID-19. Clinical researchers will gain a strong understanding of the molecular basis for a particular disease and better understand future directions for research in this still-growing field. A comprehensive reference ranging from basic biochemistry, cell biology, and physiology principles to the clinical diagnostic and management implications of vitamin D Saves researchers and clinicians time in quickly accessing the very latest details on the diverse scientific and clinical aspects of vitamin D, as opposed to searching through thousands of journal articles Chapters written by the most prominent and well-published names in the field

Vitamin D exerts its principal actions on bone metabolism, so it has important benefits on the skeleton. Serum 25(OH)D is directly related to bone mineral density (BMD), so subjects with lower levels have lower BMD and higher prevalence of osteoporosis and fractures, mainly hip and non-vertebral fractures. But, vitamin D has also many other beneficial effects, and its deficit has been associated with a great variety of diseases, such as asthma, cancer, diabetes, hypertension and other cardiovascular diseases, some inflammatory and autoimmune diseases, infections and some liver diseases. It is also remarkable its direct effect on muscle strength, so patients with vitamin D deficiency have higher risk of falls. Supplementation with vitamin D in patients with low 25(OH)D levels has shown a favourable effect not only on bone and muscle, reducing the risk of fracture, but also on inflammation, cell proliferation or immune system, reducing the risk of other diseases and complications. However, observational studies are needed with larger numbers of patients and well-designed randomized clinical trials, with baseline vitamin D determination and accurate monitoring to establish a cause-effect relationship between vitamin D deficiency and some diseases.

Many things have been said and written on the skeletal and non-skeletal effects of vitamin D, but the largest recent interventional studies are generally negative. How should we thus position ourselves in 2020? Should we say “stop” or “not yet”? Indeed, the aging of the population, as well as new challenges and discoveries, is still triggering interest in this old molecule. In this Special Edition of Nutrients, we invited top experts to give their opinion on this important debate. We also encouraged scientists to submit their latest research on nutritional requirements in the general population and in high-risk groups, as well as treatment strategies, epidemiology, analytical updates and new devices for vitamin D measurement and the effect of vitamin D on bone and extra-skeletal health. As readers will see, this Special Issue reinforces the high prevalence of vitamin deficiency and insufficiency in the general population and supports the safety of this low-cost molecule, revealing new perspectives regarding the extra-skeletal effects of vitamin D.

Despite numerous reports of the association of vitamin D with a spectrum of development, disease treatment and maintenance of health, vitamin D deficiency is common. Severe vitamin D deficiency can cause rickets in infants or children and osteomalacia in adults. However, this is currently rare in industrialized countries. Nevertheless, subclinical vitamin D deficiency is more common and has been reported to be associated with osteoporosis and a higher incidence of falls or fractures. It has been reported that 96% of children with rickets were breastfed, as breast milk contains insufficient vitamin D. Since vitamin D receptors are present throughout the body, inadequate vitamin D status may be associated with several extra-skeletal effects, such as pregnancy-related complications and immunodeficiency. It is well known that vitamin D can be obtained by exposure to sunlight or natural food sources. The American Academy of Dermatology has classified ultraviolet radiation as a known skin carcinogen. Therefore, it may not be safe or effective to receive vitamin D by sun exposure. Hence, many paediatricians and physicians recommend adequate vitamin D supplementation to achieve optimal plasma concentrations. Most of the population suffers from vitamin D deficiency, especially in the winter months. Studies have reported that vitamin D deficiency may play a role in the pathogenesis and / or progression of multiple diseases. The widespread lack of vitamin D therefore deserves consideration, including raising public awareness and health professionals. This book summarizes important mechanisms of action of vitamin D in the human body based on current scientific findings.
