Abstract: Introduction: Oral diseases such as dental caries and periodontal diseases are among the most common diseases in Hong Kong. Family as a basic societal unit has an important role to play in shaping the individual member's behaviours and health status. However, most dental research studies focus on modifiable risk factors of individuals rather than those of a family. Objectives: 1. to describe the oral health behaviours and oral health status of family members in a selected sample of families in Hong Kong; 2. to assess the relationships between oral health behaviours and oral health status amongst the family members; 3. to assess the influence of socio-economic factors, lifestyle factors and oral health knowledge, attitudes, and behavioural factors on oral health status of the spouses; and 4. to assess the influence of parental factors on the child's oral health behaviours and status. Methods: A cross-sectional survey using a combination of a random household and a purposive sampling was conducted. The study population was 5 to 7-year-old children and their parents in Hong Kong. A clinical examination and a questionnaire survey were conducted on the core family members (parents and children) of the recruited families. Structural Equation Modeling (SEM) was employed to test the hypothesized multivariate models which tried to investigate the complex relationships among different risk factors and oral health status. Results: A total of 432 families with targeted children, 373 fathers and 424 mothers were recruited. The mean DMFT/dmft score of the father, the mother, and their children were 7.2, 6.2, and 2.6, respectively. Around half (52%) of the fathers and one-third (35%) of the mothers had periodontal probing pocket(s) deeper than 3mm. In the structural equation models, strong positive correlations were found between the oral health behaviours of fathers and mothers (∅=0.98, pConclusion: Oral health behaviours and status are correlated among family members. Children's oral health status is affected by their oral health behaviours, which may be affected by parents' socio-economic status, oral health knowledge, attitudes, and behaviours. Purpose: To evaluate the relationship between demographics, dental beliefs, and oral hygiene practices and endorsing oral health fatalism beliefs among caregivers of children seeking dental care. Methods: Study subjects were English-speaking primary caregivers of children presenting for dental care at four locations: a children’s hospital dental clinic, its outpatient dental surgery center, and two private pediatric dentistry practices in the Columbus, OH metro area. A 33-item questionnaire distributed to participants which queried about gender, race, education level, insurance type, number of children, general fatalistic views, oral health knowledge and beliefs, and oral health status of the caregiver or the child. Participants were asked to rate their agreement to the oral health fatalism (OHF) statement: “Most children eventually develop dental cavities.” Results: During the study period, 332 surveys were collected, 199 from the hospital dental clinic, 52 from the dental surgery center, and 48 and 33 from the two private practices. More than half (58.4%) of caregiver respondents were Caucasian with a high school or higher education (94%) (67.5%), and were raising two children. Less than 30% endorsed the OHF statement and 42.5% were neutral. After controlling for sociodemographic factors, those caregivers with Medicaid insurance had higher OHF versus those who had other insurance (AOR = 2.21, CI 95% 1.16-4.35, p=0.02). Caregivers who described their child’s overall dental health as fair (AOR = 3.28, CI 95% 1.43-8.11, p=0.01) or poor (AOR = 3.36, CI 95% 1.11-10.36, p=0.03) had higher OHF fatalism than those caregivers who described it as excellent. Caregivers reporting it was their child’s first visit to the dentist had higher OHF than those who reported they attend dental visits once or twice a year (AOR = 2.83,CI 95% 1.10,7.31, p=0.03). Caregivers that reported their child never had a cavity were less likely to have high OHF (AOR = 0.43, CI 95% 0.21,0.84, p=0.02). Conclusions: Higher endorsement of oral health fatalism was associated with caregivers of children utilizing Medicaid versus those not, caregivers who described their child’s overall dental health as fair or poor versus excellent, first dental visits compared to those reporting regular dental care, and general fatalistic beliefs. Caregivers that reported their child never had a cavity were less likely to have endorse oral health fatalism. Future studies will be needed to show if fatalism can be changed over time and the role providers play.

The dentist must become a true health professional. “He will stop taking care of teeth and take care of people.” (Weyne, 1999) This module aims to encourage reflection on the importance of oral health in the practice of family care. According to the National Primary Care Policy, the practice of extended family care proposes interventions that influence the health-disease processes of individuals, families and the community (India, 2006). From this perspective, it values differences in the realities of families in relation to their coexistence, interaction and subjectivity. The family life cycle characterizes the movements and transformations that occur throughout family history, presenting patterns of relative predictability, constituting stages of development and requiring adaptation and adjustment from its members (Moysés and Silveira Filho, 2002). Furthermore, each family member, by playing a role according to their social context, can influence the dynamics of the health-disease process. The expanded understanding of health promotion, which includes actions aimed at changing behavior and preventing diseases, as well as the social determinants of health of populations, implies a different approach in the work of professionals in the oral health team. In this sense, it is essential to prepare oral health professionals in practices and attitudes that help them in this process. To this end, professionals must know the broader issues of the community, as well as participate in them, with the aim of educating and informing people about the effects of diet, hygiene, use of fluorides, smoking, alcohol consumption and the health/health process. illness. In this way, we will help individuals to have a healthier lifestyle based on understanding the life cycles present in families. In the Family Health Strategy (FHS), understanding the family life cycle and incorporating it in the construction of health care strategies means incorporating the family's impact on the health and illness of its members, better understanding the moment and context of the family life (Moysés and Krieger, 2008). In this module, care strategies will be addressed in relation to families' oral health, including the different stages of the life cycle: baby, child, teenager, pregnant woman, adult, elderly person and patient with special needs.

Dental caries is the most common chronic childhood disease. Chinese immigrant children have higher decayed and filled primary teeth than their US counterparts. There is limited information regarding Chinese American immigrant families' oral health beliefs and practices, which could influence their children's oral health outcomes. This study generated new knowledge about parental oral health beliefs and practices toward early childhood caries among Chinese immigrant parents toward the long-range goal of improving immigrant children's oral health. Aims. The overall goal of this cross-sectional descriptive study was to describe Chinese immigrant parents' Explanatory Model of dental caries (etiology, onset of symptoms, pathophysiology, course of caries, treatment) and the impact of Chinese culture, Chinese health beliefs and behaviors on parents' oral-health care of their children. Methods. Purposive and modified respondent-driven samplings were used to recruit Chinese immigrant parents of children aged 0-6 years through Chinese immigrant agencies and participant referrals in King County, Washington. Participants completed in-person, semi-structured interviews that included perceived influencing factors in order to capture cultural and personal perspectives about early childhood caries and children's oral health care. Interviews were audio-recorded, transcribed verbatim in Chinese and analyzed using content analysis. Results. Forty-six parents participated in the study. Parents believed the causes of dental caries included diet and oral hygiene habits, biological and somatic factors (inheritance, genetic, inborn and transmission issues), children's biting fingers habit, and lack of routine dental visits. They held different understandings on the age of onset for dental caries. Most parents recognized the onset of disease through dental examinations, children's toothaches, and color changes on their children's teeth. Parents were familiar with dental caries through personal experiences and experiences with their friends' children. Parents believed childhood caries had the potential to have a short-term or lifelong negative impact on their children. Most parents would immediately seek professional help for caries treatment, but indicated that information about dental caries and its prevention was needed from dental professional. Furthermore, results from the analysis using a framework derived from a critical analysis of Chinese culture revealed that 11 concepts were reflected in parents' oral-health beliefs and practices. Parents believed that they: (1) had an obligation for their children's oral care; (2) valued their children's training in oral care; (3) expected children to be independent in tooth brushing; (4) allowed children to receive sweets as gifts from relatives; (5) expressed fatalism regarding children's dental caries; (6) believed kinship contributed to children's dental caries; (7) obeyed dentists' authority; (8) avoided confrontation with dentists and family members; (9) demonstrated group orientation related to friends' dental knowledge and advice; (10) valued face related to children's physical facial appearance; and (11) were concerned dental caries would affect their children's school achievement. Results from the analysis using a framework derived from a critical analysis of Chinese health beliefs and behaviors revealed that eight domains were related to their oral-health beliefs and practices, including (1) belief regarding primary teeth; (2) belief regarding tooth worms; (3) belief regarding gum bleeding; (4) belief regarding sugary foods; (5) belief regarding a balanced diet; (6) belief regarding self-care; (7) belief regarding the need for dental care services; and (8) belief regarding tooth extraction. Implications. Explanatory Model of dental caries could be applied to assess parental beliefs and practices towards childhood dental caries. Understanding of Chinese culture, health beliefs and behaviors could be helpful in developing oral-health interventions in relation to family needs. Caries prevention intervention is needed to facilitate parents' daily assistance roles in children's oral hygiene behaviors, as well as healthy diet habits. The knowledge served as a foundation for developing a conceptual framework for designing culturally relevant family-focused interventions to improve immigrant children's oral health.

Access to oral health care is essential to promoting and maintaining overall health and well-being, yet only half of the population visits a dentist each year. Poor and minority children are less likely to have access to oral health care than are their nonpoor and nonminority peers. Older adults, people who live in rural areas, and disabled individuals, uniformly confront access barriers, regardless of their financial resources. The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care. These recommendations support the creation of a diverse workforce that is competent, compensated, and authorized to serve vulnerable and underserved populations across the life cycle.
