The purpose of this study was to identify the self-perceived skills and competencies of selected newly appointed middle managers. The target population was the first time, first-line, nurse manager in the 0-3 year range. The study was conducted in one urban hospital. This was an explorative, descriptive study utilizing a questionnaire instrument. This questionnaire identifies the skills and competencies the new manager feels they can complete independently and the areas in which they feel the need for additional assistance. The results indicated that there are some definite areas where the first time, first-line, manager could benefit from more specific educational preparation. These areas included problem identification, planning and evaluating the care provided; the development of retention strategies and formal orientation programs; and budgeting, specifically, controlling expenses, staffing/scheduling and coordinating/directing.

The purpose of this descriptive research was to explore leadership competency of first-line nurse managers in Shanghai, China. A total of 20 experts participated in this study including 5 directors of nursing, 12 head nurses, 2 senior nurses and 1 nursing educator. The Delphi technique consisted of three steps. First, the experts described the leadership competency of first-line nurse managers in Shanghai. Second, analyzed data using content analysis to develop rating scales. All items in the second round questionnaire met the criteria of consensus. The acceptable criteria of consensus were median equal to or greater than 3.50 and interquartile range equal to or less than 1.50. Third, developed the third round questionnaire and sent to the previous experts to confirm the items. Data was analyzed using median and interquartile range. The results showed that leadership competency of first-line nurse managers in Shanghai, China consisted of 8 components and 60 items as follows. 1) Personal mastery, 11 items; 2) systems thinking, 9 items; 3) communication and relationship building, 11 items; 4) change management, 7 items; 5) human resource management includes 9 items; 6) financial management, 5 items; 7) information technology, 4 items; 8) knowledge of the healthcare environment, 4 items.

Concerns related to the role of nurse managers in Ireland were identified by the Commission on Nursing who recommended a framework to address these concerns paying particular attention to the role of first line nursing and midwifery management. The combination of managerial functions and clinical/professional leadership required of first line managers calls for a significant level of managerial competence together with clinical credibility. Four functions of the first line nurse manager were identified as professional/clinical leadership, staffing and staff development, resource management and facilitating communication. It was further recommended that first line managers have management training before taking up a post and be required and supported in continuing to develop management skills which should operate in tandem with maintaining clinical credibility by being aware of changes in clinical practice. The Commission finally recommends that there be three grades of Clinical Nurse Manager but fails to provide role clarification/boundaries for the three grades of Clinical Nurse Manager. This study clarifies the role of the CNM2.
