Background: Dental implant placement using flapless surgery is a minimally invasive technique that improves blood supply compared with flapped surgery. However, the flapless technique does not provide access to allow bone regeneration. Objective: The aim of this systematic review was to evaluate the clinical parameters following implant surgery in healed sites, using two procedures: flapped vs. flapless surgery. Materials and methods: A detailed electronic search was carried out in the PubMed/Medline, Embase and Cochrane Library databases. The focused question was, u201cHow do flapped and flapless surgical techniques affect the clinical parameters of dental implants placed in healed sites?u201d. All the studies included with a prospective controlled design were considered separately, depending on whether they had been conducted on animals or humans. The following data were recorded in all the included studies: number of implants, failures, location (maxilla, mandible), type of rehabilitation (partial or single), follow-up and flap design. The variables selected for comparison in the animal studies were the following: flap design, gingival index, mucosal height, recession and probing pocket depth. In humans studies the variables were as follows: flap design, plaque index, gingival index, recession, probing pocket depth, papilla index and keratinized gingiva. Results: Ten studies were included, six were experimental studies and four were clinical studies. Studies in animals and humans showed better results using the flapless technique in all the parameters analyzed. Conclusion: Implants placed in healed sites with a flapless approach have a significant advantage in clinical parameters over the flapped procedure in a short term follow-up.

This book demonstrates the effect of flapless versus flap surgery upon peri-implant soft and hard tissues. The implant insertion is done using a stereolithographic surgical guide to provide a precised insertion into the jaw bones to avoid misdirection complications. The changes of peri-implant soft tissue profile surrounding an implant are evaluated utilizing a variety of soft tissue parameters after a period of fifteen days, 3 months, 6 months and twelve months. In addition, crestal bone changes on the mesial and distal aspects of the implant are recorded with the aid of digital x-rays taken immediately after the surgery, fifteen days, 3 months, six months, and twelve months. The dental implant is loaded after a period of 3 months from since the time of surgery. According to our results, it is found that flapless implant surgery provides better outcomes regrading the soft tissue conditions in contrast to the flap surgery.

BackgroundIt is already demonstrated that one step approach immediate after teeth extractions have lots of advantages upon two-steps one. A widely discussed theme remains the necessity of grafting the buccal space between implant and cortical plate, especially in cases when the buccal wall of the socket is intact.AimTo appreciate the influence of the grafting material usage in case of one step flapless placement of two piece dental implants immediate after teeth extractions in esthetic zone.Matherial and methodsNinety five two-piece dental implants (Dentium superline) were inserted in 44 patients (46u00b11,74 years) after teeth extraction in the esthetic zone using one step flapless approach. All the implants have been placed close to the palatal bone, creating a space between the buccal bone and implants surface. In the study group, 49 implants were placed without grafting materials. In the control group, 45 implants were installed and collagen/hydroxyapatite grafting material were used to fill the buccal space. In both groups, when insertion torque shifted 30 Ncm, temporary crown were delivered. According to the orthopantomogram, implantsu2019 sides were divided into anterior and posterior ones. The following parameters were evaluated: primary and secondary stability (Periotest Classic), peri-implant bone modeling (1 year follow-up).
