There are many textbooks on fractures in adults, but few deal with those in children and young persons. Publications on fractures in children are usually written by orthope dic surgeons who are familiar with the problems presented by the growing skeleton. The principles taught in the best known works on fractures in children by BLOUNT (1954), RETTIG (1957), CHIGOT and ESTEVE (1957), EHALT (1960), POLLEN (1973), and RANG (1974), differ from each other only slightly. The controversy which has sur rounded the treatment of fractures in adults in recent years has never extended to chi1drens' fractures. Skillful nonoperative treatment of the latter leads to uneventful healing in the majority of cases, which cannot always be said of similar injuries in adults. "Considerable skill" is required to induce nonunion in a child, and its occur rence always results from a serious error of management. In the adult, prolonged immobilization frequently leads to muscle atrophy, joint stiffness, and disturbances of the peripheral innervation and circulation. Fracture treatment by internal fixation in the manner originally described by LANE (1894), LAMBOTTE (1892, 1907, 1913), KONIG (1905, 1931), and DANIS (1932, 1949) has gained wide acceptance only in the last 15 years. Since 1958, MULLER, ALLGOWER, WIL LENEGGER, and the other members of the Swiss Association for the Study of Internal Fixation (ASIF) have been developing techniques and instruments for the operative treatment of fractures.

The treatment of humeral fractures is a complex issue and the source of considerable controversy. In the case of fractures of the proximal humerus, early range of motion is the main aim of treatment. After the publication of the AO book Technique of Internal Fixation of Fractures (Miiller, Allgower and Willenegger, Springer-Verlag, 1965), the authors decided after considerable discussion amongst themselves and other members of the Swiss AO that the next edition would appear in three volumes. In 1969, the first volume was published (the English edition, Manual of Internal Fixation, appeared in 1970). This was a manual of surgical technique which discussed implants and instruments and in which the problems of internal fixation were presented schematically without radio logical illustrations. The second volume was to be a treatise on the biomechanical basis of internal fixation as elucidated by the work done in the laboratory for experi mental surgery in Davos. The third volume was planned as the culminating effort based upon the first two volumes, treating the problems of specific fractures and richly illustrated with clinical and radiological examples. It was also to discuss results of treatment, comparing the results obtained with the AO method with other methods. The second and third volumes were never published. The second edition of the AO Manual appeared in 1977. It dealt in greater detail with the problems discussed in the first edition, although it still lacked clinical exam ples and any discussion of indications for surgery. Like the first edition, it was trans lated into many languages and was well received. Finally, after 22 years, the much discussed and much needed third volume has appeared.

The purpose of this book is to report on my 17 years of experience in the development and implementation of a closed functional method of treatment of certain fractures of long bones of the appendicular skeleton. My personal experiences, clinical results, and the basic concepts underlying the treatment philosophy are reported. Illustrations demonstrating the techniques of application of functional casts and braces are presented in detail as well as a step-by-step description of the management regime and a discussion of the behavior of fractures under this system. Indications and contraindications for the use of this method of treatment are clearly outlined. I consider it important to compile this information for I believe that the system has withstood the test of time and is sufficiently sound to be incorporated in the armamentarium of the orthopaedic surgeon. Also, it may have a favorable socioeconomic impact since it offers a viable alternative in the treatment of frac tures.
