Bätzing, 2009, S. 38; Kruse / Körkel / Schmalz, 2000, S. 92 f.). Der Anteil der alkoholabhängigen Menschen macht etwa drei bis fünf Prozent der Gesamtbevölkerung aus und stellt damit ein großes sozialmedizinisches Problem dar (vgl. Hell et al., 2007, S. 80). Männer sind wesentlich häufiger von dieser Krankheit betroffen, doch der Anteil der Frauen hat in den letzten Jahrzehnten drastisch zugenommen. Während der Anteil der Frauen 1960 noch auf ein Zehntel geschätzt wurde, geht man heute von etwa einem Drittel aus (vgl. Vogt, 2004, S. 33). In Anbetracht der zahlenmäßigen Überlegenheit der Männer und der Zunahme alkoholabhängiger Frauen stellen sich mehrere Fragen, auf die ich in dieser Arbeit unter anderem eingehen möchte: Welche Einflussfaktoren bedingen die geringere Anzahl abhängiger Frauen? Wie kommt es zur Zunahme des Frauenalkoholismus? Warum und auf welche Weise trinken sie? Welche Frauen sind besonders betroffen? Welchen Platz nimmt die Frau im Zusammenhang mit Alkohol in unserer Gesellschaft ein? Was unterscheidet sie von den Männern? Was haben die Geschlechter gemeinsam? Welche Faktoren schützen vor einer Abhängigkeit, welche bergen Risiken? Welche Entwicklung ist derzeit beim Alkoholkonsum von Frauen zu beobachten? Welche Besonderheiten ergeben sich für die Behandlung alkoholabhängiger Frauen? (...) Ich werde zunächst in Kapitel zwei erläutern, was unter Alkoholismus zu verstehen ist. Alcohol and nicotine addiction mostly occur together. Over the last ten years therapeutic aspects and motivational strategies have been considerably improved. The association between social class and psychiatric disorder has been one of the central topics of socio-epidemiological research since its inception. With remarkable consistency, numerous investigations have demonstrated an inverse correlation between social class and prevalence rates for most forms of psychopathology. The debate on the interpreta tion of these findings - social causation versus social selection processes - continues to this day. Moreover, the question as to what the psychoso cial processes are through which social class and individual psychopa thology are mediated has remained mostly unanswered. The concept of social stress may well provide new insights in this regard. One should note, in particular, the considerable conceptual and methodological progress made in life event research. Recently, the first attempts have been made to link the two concepts of social class and social stress. These developments will be retraced in this book in the light of new investigations. Descriptive epidemiological studies on the relation between social class and psychiatric disorders form the point of depar ture. Explicit reference is made to two classical studies in psychiatric epidemiology: Faris and Dunham's Chicago study and the New Haven study by Hollingshead and Redlich. Following on from there, various approaches in analytical epidemiological research are presented which attempt to determine whether - or, more precisely, to what extent - social causation or social selection processes can be said to be responsible for the higher rates of psychiatric disorders in lower social strata.

Over the past 200 years, a health reform movement has emerged about every 80 years. These clean living cycles surged with, or were tangential to, a religious awakening. Simultaneously with these awakenings, out groups such as immigrants and/or youth were seen to exhibit behaviors that undermined society. Middle class fear of these dangerous classes and a desire to eliminate disease, crime, and other perceived health or social problems led to crusades in each of the three reform eras against alcohol, tobacco, drugs, certain foods, and sexual behaviors. A backlash began to emerge from some segments of the population against reform efforts. After the dissipation of the activism phase, laws made during the reform era often became ignored or repealed. With a few exceptions, during the 30 to 40 year ebb of the cycle, the memory of the movement disappeared from public awareness. The desire for improved health and social conditions also led to campaigns in favor of exercise, semi-vegetarian diets, women's rights, chastity, and eugenics. Engs describes the interweaving of temperance, women's rights, or religion with most health issues. Factions of established faiths emerged to fight perceived immorality, while alternative religions formed and adopted health reform as dogma. In the reform phase of each cycle, a new infectious disease threatened the population. Some alternative medical practices became popular that later were incorporated into orthodox medicine and public health. Ironically, over each succeeding movement, reformers became more likely to represent grass roots beliefs, or even to be state or federal officials, rather than independent activists.
