Globalization is characterized by a systemic interconnection in which what hapInequality has been on the rise across the globe and living conditions are vastly unequal between different places in the world. Currently, the richest 1% own 45% of the world’s wealth. The consequence is that some people are able to enjoy healthy, wealthy, happy lives whilst others continue to live in ill-health, poverty and sorrow. Rapid economic growth in Asia (particularly China and India) has lifted many people out of extreme poverty. Nevertheless, the wealth divide is steadily growing. According to Oxfam, between 2009 and 2018, the number of billionaires it took to equal the wealth of the world’s poorest 50 percent fell from 380 to 26. Those with extreme wealth have often accumulated their fortunes on the backs of people around the world who work for poor wages and under dangerous conditions. Women are scarce at the top and overrepresented at the bottom. Gender discrimination in the workplace contributes significantly to these persistent economic divides. There are also large differences in wealth across racial groups. Long-standing racial discrimination in many forms, including in education, hiring, and pay practices contribute to persistent earnings gaps. Inequalities have dramatically strengthened the economic and political power of those individuals at the top.

Disabled children often have feeding difficulty. Choking, food spillage and protracted mealtimes can pose enormous problems for their carers and the accompanying nutritional deficit imposes additional burdens on the affected child. The aetiology of these problems is placed in context by a detailed description of normal feeding development in infants. The often under-recognised nutritional and neurodevelopmental consequences of inadequate nutrient intake together with the respiratory complications and the important problems of constipation and drooling which may accompany oro-motor dysfunction are detailed. The clinical and nutritional assessment and the application of diagnostic imaging techniques in the evaluation of such children are described. Despite the fact that the average woman spends one third of her life after the menopause, medical research has been devoted almost entirely to the repro ductive period of her life span. This is perhaps not surprising in our youth orientated society and yet there is increasing evidence that properly applied and supervised hormonal therapy could alleviate many of the severe physical symptoms which are associated with the ovarian menopause and that in the long term other aspects of physical deterioration could be modified. This lack of scientific research has made it difficult to assess which symptoms are due to the altered hormonal status of the post-menopausal period and which are due to the normal process of ageing, or the various psychological pressures which build up around most women in the fourth and fifth decades of life. In America doctors have been treating the 'menopausal syndrome' with estrogens for over 30 years, but in the United Kingdom gynaecologists and family doctors have been reticent to prescribe these steroid preparations. As a consequence, they have been labelled reactionary by the media and while there may be some truth -in this, it should be remembered that the hazar. ds associated with synthetic estrogens in the contraceptive pill were first brought to light by British epidemiological surveys.
