"Although the U.S. aviation system isone of the safest in the world,hundreds of fatalities occur each yearin general aviation-which includes allforms of aviation except commercialand military. The general aviationindustry is composed of a diverse fleetof over 220,000 aircraft that conduct awide variety of operations-frompersonal pleasure flights in small,piston aircraft to worldwideprofessionally piloted corporate flightsin turbine-powered aircraft. Accordingto 2011 National Transportation SafetyBoard (NTSB) data, 92 percent of thatyear's fatal accidents occurred ingeneral aviation. The majority ofgeneral aviation accidents areattributed to pilot error.GAO was asked to examine the (1)characteristics of and trends in generalaviation accidents from 1999 through2011 and (2) recent actions taken byFAA to improve general aviationsafety. GAO analyzed NTSB accidentdata, reviewed government andindustry studies and other documents,and interviewed FAA and NTSBofficials and industry stakeholders."

This work examines the causes of airplane accidents and what private and public policies are needed to improve aviation safety. It begins by examining the safety record of the United States commuter airline industry in the post-deregulation era characterized by increased emphasis by airlines on cost control and growing pressures on the air traffic control and airport system. The authors go beyond the safety of the scheduled airlines to examine the reasons for accidents in the nonscheduled and general aviation segments of the United States industry, where the bulk of fatalities occur and where airline pilots increasingly receive most of their training and experience. They then turn to an examination of aviation safety throughout the world, first with a detailed comparison of Canadian and American aviation safety, and then with a look at air safety in all regions of the world and the safety performances of all the world's major airlines. Pursuant to a congressional request, GAO reviewed the Federal Aviation Administration's (FAA) oversight of general aviation safety, focusing on the: (1) extent to which mountainous areas present higher risks than nonmountainous areas for general aviation; and (2) FAA actions to reduce the risks associated with mountain flying and the impact of those actions on general aviation. GAO also examined the legal and safety issues involved with the prohibition imposed on general aviation night operations at Aspen Airport. GAO found that: (1) mountain flying poses greater accident risks to general aviation pilots; (2) engine output and propeller efficiency decrease at higher elevations; (3) FAA alerts pilots to mountain flying risks during the pilot certification process and at subsequent safety seminars; (4) pilots involved in fatal general aviation accidents were not familiar with the hazards of mountain flying; (5) FAA should issue guidance identifying airports in mountainous areas that present unique challenges and recommend routes for approach and takeoff at those airports under visual flight rules; (6) FAA should encourage pilot training by approving mountain flying courses and allowing pilots who complete such training to receive a "mountain endorsement" that can be used in lieu of the biennial flight review requirement; (7) FAA should develop certification test questions that highlight mountain flying risks, since the current test does not include any; (8) FAA has legal authority to resolve the dispute concerning the restriction of night operations at Aspen Airport; and (9) FAA has several options to consider in resolving the dispute at Aspen Airport.
