In January 1995 the Institute of Medicine released a preliminary report containing initial findings and recommendations on the federal government's response to reports by some veterans and their families that they were suffering from illnesses related to military service in the Persian Gulf War. The committee was asked to review the government's means of collecting and maintaining information for assessing the health consequences of military service and to recommend improvements and epidemiological studies if warranted. This new volume reflects an additional year of study by the committee and the full results of its three-year effort.

DOD-funded GWI research began in 1994 with the establishment of a Gulf War Veterans' Illnesses Research Program (GWVIRP) to study the health effects of service members deployed in the 1990-1991 Persian Gulf War. From FY94 to FY05, the GWVIRP was managed by the USAMRMC Military Operational Medicine Research Program (MOMRP). Research pertaining to GWI also was funded intermittently through the CDMRPs Peer Reviewed Medical Research Program (PRMRP), which supports military health-related research. The MOMRP shared management responsibility for the GWVIRP with the CDMRP in FY06 with separate $5 million appropriations. Although the GWVIRP did not receive funding in FY07, a $10 million appropriation renewed the program in FY08, renamed the Gulf War Illness Research Program (GWIRP), to be managed fully by the CDMRP. The GWIRP has continued in FY09 and FY10 with an $8 million appropriation each year. The program is directed to support peer reviewed research of treatments for the complex of symptoms that comprise GWI, identification of objective markers (biomarkers) for the disease, and understanding the pathobiology underlying GWI.

This report was produced by the federal Research Advisory Committee on Gulf War Veterans' Illnesses (RACGWVI), which was established by Congress under Public Law 105-368. Membership includes veterans of the 1990-91 Gulf War, scientists who have studied illnesses affecting these veterans, clinicians who care for ill Gulf War veterans and a member of the general public. The Committee periodically releases reports that summarize research to date on the health of veterans of the 1990-1991 Gulf War. The most recent report was published in 2008, and the current report updates knowledge from that time by reviewing published scientific papers that appeared after the last report and through December 2013. The present research review is divided into four sections. The first summarizes the new information available on rates of Gulf War illness and other illnesses and disabilities that affect groups of veterans from the Gulf War (Section 1, Epidemiologic Research). The second reviews the human and animal research that has been carried out to identify the causes of Gulf War illness and other health problems in Gulf War veterans (Section 2: Etiologic Investigations). The third section focuses on studies of the disruptions in normal body functions that underlie the symptoms of Gulf War illness and other health problems (Section 3: Pathobiology of Gulf War illness). And the fourth reviews clinical trials that are underway to treat Gulf War illness (Section 4: Gulf War illness treatment research).
