Die beschriebene Behandlungsweise reicht fur die Mehrzahl der Falle von Typbus aus. Aber es gibt zahlreiche Falle, welche ein differentes Verfahren fordern, und in welchen nur durch energische therapeutische Massregeln drohende Gefahren abgewandt werden konnen. Die grosste Gefahr fur die Kranken liegt beim Abdominaltyphus wie bei dem exanthematisehen Typhus und bei anderen Infectionskrankheiten in der Heftigkeit des Fiebers. Aber gerade gegen diese Gefahr besitzen wir auch die wirksamsten Mittel. Die Gefahr, welche das Fieber fur den Kranken bringt, ist eine doppelte. Einerseits fuhrt die Steigerung der Korperwarme uber eine bestimmte Hohe zur Paralyse des Herzens, und macht das Fortbestehen des Lebens unmoglich, andererseits fuhrt eine anhaltende Steigerung der Warmeproduction oder, was dasselbe ist, ein anhaltend vermehrter Umsatz der Gewebe, auf welchem die fieberhafte Ueberheizung des Korpers beruht, zur Consumption der Kranken. Bei fieberhaften Krankheiten von verhaltnissmassig kurzer Dauer, z. B. bei den acuten Exanthemen, bei dem exanthematischen Typhus, bei dem Abdominaltyphus, ist die Gefahr, welche in der Steigerung der Korpertemperatur liegt, selbstverstandlich mehr zu furchten, als die, welche die Steigerung der Warmeproduction mit sich bringt. Aber dennoch darf man auch die letztere nicht aus dem Auge verlieren, wenn man die erste bekampft, weil man sonst dem Kranken, anstatt ihm zu nutzen, schaden konnte. Excerpt: ...mit Lebersyphilis syphilitische Schwielen im Pankreas, mit oder ohne Gummata; auch Lancereaux versichert, in mehreren Fallen von visceraler Lues das Pankreas hart, indurirt, sklerosirt gefunden zu haben, und meint, man wurde derartigen Zustanden wohl haufiger begegnen, wenn man das Pankreas regelmassig bei Sectionen untersuchen wurde. Beim Neugeborenen und Fotus hat man mehrfach unzweifelhafte Degenerationen des Pankreas als Theilerscheinung hereditarer Syphilis constatirt. Hierher gehoren ohne Zweifel die von Cruveilhier bei zwei Neugeborenen gefundenen und abgebildeten Falle von Pankreasentartung; in dem erstereu derselben (6. Observ.) fand sich bei dem bald nach der Geburt verstorbenen Kinde kasige Entzundung der Thymus und eine speckartige Schwellung des ganzen Pankreas. Letzteres, welches mit der rechten Niere und Nebenniere verwachsen war, zeigte auf dem Durchschnitt ein homogenes, derbes, gelbweisses Gewebe ohne jede Spur eines acinosen Baues, welches Cruveilhier mit dem Tissu squirrheux" vergleicht. Oedmansson fand bei hereditarer Syphilis mitunter Veranderungen des Pankreas, bestehend in allgemeiner Fettdegeneration der Drusenzellen und bedeutender interstitieller Hyperplasie, und Klebs beschreibt mehrere Gummiknoten im Pankreas neben syphilitischen Veranderungen der Lungen, Leber und Nieren bei einem sechsmonatlichen Fotus. Neuerlichst hat Birch-Hirse hfe 1 d unsere Kenntnisse durch den Nachweis des keineswegs seltenen Vorkommens interstitieller Bindegewebswucherungen im Pankreas bei hereditarer Syphilis der Neugeborenen (13 mal unter 23 Fallen) bereichert.
