The research in this technical brief is part of a multi-country study commissioned by the Alliance for Health Policy and Systems Research to understand the evidence-to-policy process around health decisions in Ghana, Ethiopia and Mozambique. Through surveys and in-depth interviews with representatives from 54 locally based research institutions and 41 health policy-makers, the studies explored the entire chain of evidence generation, from conception of the research idea, through to its synthesis and dissemination. For each country, this included discussions with key actors involved in tracer policies. This technical brief distils regional lessons from the studies and offers a set of recommendations to support efforts to develop and strengthen existing institutions towards enhanced uptake of research.

The challenges to better health services in Africa are well known: Africa lags behind all regions of the world, including other developing regions, on all indicators of better health. A recent report from the World Health Organisation for instance shows that while Africa has 20% of the world's sick people, it has only 4% of its healthcare workers - many of them vulnerable to the high mortality rate associated with malaria and notably the AIDS epidemic. The state of investment in healthcare infrastructure is also grossly inadequate as is the efficiency of healthcare delivery. But does this need to be so? What factors are responsible for this unacceptable state of affairs? Contributors to the volume examine the evolution of healthcare services in Africa, the ongoing national, regional and continental efforts to improve the delivery of healthcare in the continent, and the direct and indirect obstacles militating against the maturation of the services and their efficient delivery. The contributors - all distinguished experts in the field, who hold either challenging responsibilities in health in Africa or have worked in multiple components of the healthcare delivery system in the continent - also provide powerful personal insights and lessons learned in their current or previous work in the health sector in Africa. A large proportion of the world's low- and middle-income population reside in sub-Saharan Africa where out-of-pocket payment systems have made healthcare inaccessible and unaffordable in many places. Universal health coverage (UHC) means that all people have access to high-quality health services and are protected against financial-risk while using those health services. Financing approaches to UHC include varieties of health insurance and tax-funded systems. National Health Insurance (NHI) is one such financing mechanism established by a national government with the goal of covering all or almost all citizens of the country. This descriptive, qualitative study used policy sources from four countries, Ghana, Kenya, Zambia, and South Africa, that are implementing or transitioning to an NHI scheme. The research goal was to understand how the introduction of an NHI scheme might affect national health policy making and to use the results to inform South Africa and other countries in sub-Saharan Africa that are introducing NHI. A conceptual framework of 16 NHI policy domains was developed. Data were extracted from policy sources and indexed according to the different policy domains. Results revealed that following passage of legislation, NHI bodies were making policy decisions in order to operationalize and manage implementation of the NHI. We found variation in the four countries in terms of the types of policies developed and the decision-making authority around those policies. A key finding was that that Ministries of Health retained decision-making power over the NHIs through regulations and appointment of board members. However, NHIs were often delegated policymaking authority in key areas including financing mechanisms, provider payments, member payments, benefit schemes, accreditation, and relationships with private health insurance schemes. This study helps fill an important gap in research concerning policymaking responsibility in a health system after an NHI is adopted. Lessons from the focus countries can inform the transition to an NHI scheme including introduction of policies and delegation of decision-making authority in countries in sub-Saharan Africa considering NHI as a UHC financing mechanism.

[...] a powerful equalizer that abolishes distinctions between the rich and the poor, the privileged and the marginalized, the young and the old, ethnic groups, and women and men (Chan 2012). [...] Depth of coverage corresponds to the number and quality of services and assesses the range of health services available to meet the healthcare needs of covered populations. [...] Strong political leadership also featured in the cases of South Africa and Nigeria, although for the former, the apartheid root of the health insurance system deprived the scheme of equity as a key UHC objective, which has not been redressed in the post-apartheid period.

In 2011 the Grand Challenges in Global Mental Health initiative identified priorities that have the potential to make a significant impact on the lives of people with mental, neurological, and substance use disorders. Reduction of the cost and improvement of the supply of effective medicines was highlighted as one of the top five challenges. For low- and middle-income countries, improving access to appropriate essential medicines can be a tremendous challenge and a critical barrier to scaling up quality care for mental, neurological, and substance use disorders. Reduction of cost and improvement of the supply of effective medicines has the potential to significantly impact the lives of patients with these disorders. Improving Access to Essential Medicines for Mental, Neurological, and Substance Use Disorders in Sub-Saharan Africa is the summary of a workshop convened by the Institute of Medicine Neuroscience Forum in January 2014 in Addis Ababa, Ethiopia to discuss opportunities for achieving long-term affordable access to medicines for these disorders. This report examines challenges and opportunities for improving access to essential medicines in four critical areas: demand, selection, supply chains, and financing and pricing. Drawing on various disciplinary perspectives, this book re-focuses the debate on what makes a good health system, with a view to clarifying the uses of social science research in thinking about health care issues in Africa. The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.
