Hepatitis A is a major health concern throughout the world. Its impact has largely been limited in recent times by the large-scale use of vaccines. It is, however, still rampant in various parts of the world, partly due to lack of medication, poor water access, and contaminated food products. This book provides comprehensive information on various aspects of hepatitis A with a focus on three of the most important biliary diseases: cholestasis, primary sclerosing cholangitis, and hemophagocytic lymphohistiocytosis. Chapters cover such topics as pathology, epidemiology, at-risk populations, animal research models, and future trends.

This book is aimed to emphasize the rationale and importance of prevention and management of viral hepatitis in children, providing cutting edge knowledge. Viral hepatitis is a major health problem in the world. Although most complications of viral hepatitis are observed in adults, primary infection with hepatitis viruses often occurs during infancy or childhood. To better control viral hepatitis, prevention and therapy if possible should be started in childhood. This book offers updated and unique information about viral hepatitis in children, which has vitally important impact on global disease outcome and control, yet not discussed as frequently as viral hepatitis in adults in previous medical literature. Better prevention and management strategies are covered, starting from infancy and childhood, and even earlier during fetal life. It will be very helpful for better control of viral hepatitis both for daily practice and for developing future strategies and directions. If we can successfully control viral hepatitis in children, there will be very little remaining chronic hepatitis and related complications such as liver cirrhosis or hepatoma in adults. We hope readers, including medical students, researchers, pediatricians, family medicine physicians, infectious disease personnel, public health workers, gastroenterologists, hepatologists and parents of children with chronic hepatitis, will be benefited by reading this book.

A series of international symposia on viral hepatitis and liver disease has been held triannially, and called the "Olympics" of this research field. Our book presents the results of the eighth of these "Olympiads" which for the first time, was held in Asia (May 1993, Tokyo). Due to the rapid progress in research on both basic and clinical aspects of viral hepatitis and liver disease, the state of the art in this field is continually being updated, and our book provides a broad and in-depth survey of current work. The major topics in our book include molecular biology of the five known hepatitis viruses (HAV, HBV, HCV, HDV, and HEV), clinical implications of genetic variants of HBV and HCV, interferon treatment of HCV-related liver disease, and worldwide epidemiology and control of viral hepatitis. New subjects not seen in previous books, such as genotypes of HCV, are also covered. Expanding knowledge about the heterogeneity of the HCV genome has revealed a great variety of genotypes as well as their association with host pathogenesis and their varying responsiveness to interferon therapy. The first promising results of efforts to develop a hepatitis C vaccine are also presented. Finally, compared with its predecessors, our book contains many more papers from Asian countries, where the prevalence of viral hepatitis and liver disease is the highest in the world.

The last few years have witnessed major developments in molecular biology which have had a significant impact on biomedical knowledge. As a result new concepts in cell biology have emerged, new etiologic agents have been discovered, and new drugs have become available. Furthermore, the world of mediators, cytokines, and humoral factors has enlarged considerably in a relatively short period and this has modified our views of diverse physio pathologic processes as well as opened up new therapeutic prospects. In addition, recent technologic advances have revolutionized our diagnostic methodology and have greatly enhanced the power of our therapeutic resources. All these factors have contributed to a substantial remodeling of both liver science and clinical hepatology, and these changes have generated the motivation for the production of the present book, which attempts to give an account of well-known clinical facts in the light of current thinking. It is the fruit of the coordinated work of experts from different countries in three continents. In a sense, it is a manifestation of the fact that inter national cooperation is becoming increasingly important not only in the expansion of science but also in its exposition. We hope that Hepatobiliary Diseases will be useful to both students and clinicians and that internists, gastroenterologists, and general practitioners will find in its contents a satifactory coverage of their field of interest.

The global epidemic of hepatitis B and C is a serious public health problem. Hepatitis B and C are the major causes of chronic liver disease and liver cancer in the world. In the next 10 years, 150,000 people in the United States will die from liver disease or liver cancer associated with chronic hepatitis B virus (HBV) or hepatitis C virus (HCV) infections. Today, between 800,000 and 1.4 million people in the United States have chronic hepatitis B and between 2.7 and 3.9 million have chronic hepatitis C. People most at risk for hepatitis B and C often are the least likely to have access to medical services. Reducing the rates of illness and death associated with these diseases will require greater awareness and knowledge among health care workers, improved identification of at-risk people, and improved access to medical care. Hepatitis B is a vaccine-preventable disease. Although federal public health officials recommend that all newborns, children, and at-risk adults receive the vaccine, about 46,000 new acute cases of the HBV infection emerge each year, including 1,000 in infants who acquire the infection during birth from their HBV-positive mothers. Unfortunately, there is no vaccine for hepatitis C, which is transmitted by direct exposure to infectious blood. Hepatitis and Liver Cancer identifies missed opportunities related to the prevention and control of HBV and HCV infections. The book presents ways to reduce the numbers of new HBV and HCV infections and the morbidity and mortality related to chronic viral hepatitis. It identifies priorities for research, policy, and action geared toward federal, state, and local public health officials, stakeholder, and advocacy groups and professional organizations.

Acute and chronic hepatitis and hepatitis-related diseases such as liver failure, liver cirrhosis and hepatocellular carcinoma are among the most important causes for disabilities and death. Hepatocellular injury is due to a variety of agents including viruses, toxins, radiation, injury and drugs but also bacteria, parasites and autoimmune reactions. This volume reviews today’s knowledge about hepatitis with emphasis on comparative aspects between hepatitis in humans and animals, but also between different etiological agents. This particular viewpoint makes the book relevant for scientists from both human and veterinary medicine, gastroenterologists, pathologists, virologists and students of human and veterinary medicine.

Infections of the liver and biliary system are not uncommon, while most of the infections are viral in nature. Globally, viral hepatitis is a major public health problem and parallels infections such as HIV and tuberculosis and is associated with significant morbidity and mortality. Rapid advances have been made with HCV therapy at a time when the community focus has shifted to global elimination. This issue highlights some of the areas of considerable advances and challenges in viral hepatitis B and C. In addition, Hepatitis E is resurfacing as an infection of significance, particularly in the Western World where there is a prevalence of chronic infections. Gastroenterologists and hepatologists often think of viral hepatitis as the sole cause of hepatic infections, but there are other infections caused to bacteria, fungi and parasites; this issue highlights these infections as well. Articles are devoted to the following topics: Global epidemiology of viral hepatitis; Update on diagnostics in viral hapatitis; Heptatitis B: Immunization and impact on natural history and cancer incidence; Heptatitis B: Current status of therapy and future therapies; Hepatitis Delta: Prevalence, natural history, and treatment options; Hepatitis C: How good is real-life data and do generics work: Hepatitis C: Does successful treatment alter the natural history and quality of life; Hepatitis E: Epidemiology, clinical course, prevention, and treatment; CMV and EBV infections of the liver; Extrahepatic manifestations of viral hepatitis; Pyogenic and amebic infections of the liver; and Fungal and parasitic infections of the liver.

Acute and chronic viral hepatitis infections are serious public health threats around the world. The different infections have different epidemiology and natural his- ries, and children play important roles in each of these. For example, children are important reservoirs for acute hepatitis A, childhood infections are responsible for most of the global morbidity associated with chronic hepatitis B, and perinatal transmission of hepatitis C continues to occur, even as the overall incidence of new infections wanes. Some non-A through E viral hepatitis infections are seen p- dominantly in infants and young children, while others have specific implications for this population. Therapeutic options for children with chronic viral hepatitis are limited when compared to those available for adults, especially for hepatitis B, and considerations given to long-term therapy have dramatic implications when dealing with the long life expectancy of these young patients. With these issues in mind, this unique volume has been created to address the special considerations regarding viral hepatitis in children. It includes the latest information and recommendations specifically directed at the pediatric population, and highlights the knowledge gaps which will need to be filled to improve our understanding of these infections and treatment of this special group. Experienced practitioners from around the world have contributed these reviews, incorporating the latest studies, the current recommendations, and the distinctive pediatric issues that shape clinical care, and will determine the research agenda for this field going forward.
