"In radiation therapy, accurate dose determination and precise dose delivery to the tumour are directly associated with better treatment outcomes in terms of higher tumour control and lower post radiation therapy complications. In the past, film dosimetry was developed into a powerful tool for external beam radiotherapy treatment verification and quality assurance. The objective of this thesis was the development and clinical application of the EBT-3GafChromicTMfilm model patient specific based quality assurance (QA) procedures in brachytherapy. The lack of clinical measurements and patient specific QA procedures (similar to that in intensity modulated radiotherapy (IMRT) delivery) in high dose rate brachytherapy (HDRBT) were the motivation to improve both the QA program of delivery and patient safety during brachytherapy procedures.Patient specific QA tools for pre-operative brachytherapy in rectal cancer developed in this thesis use a radiochromic film dosimetry system together with gamma function evaluation method to compare calculated and measured dose distributions. We also created a dedicated phantom for a brachytherapy applicator used for treatment of rectal cancer patients, which enabled us to compare calculated dose distributions to measured ones in high and low dose gradient regions. Starting from the same passing criteria used for external IMRT QA (3%, 3 mm), passing criteria for high and low dose regions were subsequently discussed. Finally, we investigated the QA system's sensitivity to source positional errors by introducing intentional and controlled mistakes on selected patient plans.Results presented in this thesis demonstrated that radiochromic film dosimetry based QA for brachytherapy can be used not only for patient specific quality assurance, but as a part of the commissioning process and periodic QA as well." --

High-dose-rate (HDR) brachytherapy is a rapidly growing technique that has been replacing low-dose-rate (LDR) procedures over the last few years in both industrialised and developing countries. It is estimated that about 500,000 procedures (administrations of treatment) are performed by HDR units annually. LDR equipment has been discontinued by many manufacturers over the last few years, leaving HDR brachytherapy as the major alternative. HDR brachytherapy techniques deliver a very high dose, of the order of 1.6–5.0 Gy/min, so mistakes can lead to under- or over dosage with the potential for clinical adverse effects. More than 500 HDR accidents (including one death) have been reported along the entire chain of procedures from source packing to delivery of dose. Human error has been the prime cause of radiation events. In the present report, the International Commission on Radiological Protection concludes that many accidents could have been prevented if staff had had functional monitoring equipment and paid attention to the results. Since iridium has a relatively short half-life, the HDR sources need to be replaced approximately every 4 months. Over 10,000 HDR sources are transported annually, with the resultant potential for accidents; therefore, appropriate procedures and regulations must be observed. A number of specific recommendations on procedures and equipment are given in this report. The need for an emergency plan and for practising emergency procedures is stressed. The possibility of loss or theft of sources must be kept in mind.

Optimization approaches for treatment planning in two novel high-dose-rate (HDR) brachytherapy techniques, direction-modulation brachytherapy (DMBT) and energy-modulated brachytherapy (EMBT), are investigated for cervical cancer and prostate cancer. Brachytherapy is a form of radiation therapy where a radioactive source is placed inside the body to irradiate the tumour internally. Conventionally, only one source is used and it is unshielded, thus providing an isotropic dose distribution. DMBT makes use of a new shielded applicator that is capable of delivering highly directional radiation distributions. In EMBT, three HDR sources, 192Ir, 60Co, and 169Yb, are used in combination to provide variety in dose profiles. To investigate the benefit of these two new techniques over conventional brachytherapy, we use an inverse planning approach to generate the treatment plans. We model the treatment planning problem as a quadratic program and use an interior point constraint generation algorithm to generate the treatment plans.
