This unique, spiral-bound handbook is compact, portable, and written with busy home health nurses in mind! Organized by body system, it offers instant advice on assessment and care planning for the disorders home health nurses are likely to encounter. Providing assessment guides for all body systems, the home environment, and the client's psychological status, it includes full care plans for over 50 illnesses and conditions most commonly encountered in the home. Each plan lists nursing diagnoses, short- and long-term expected outcomes, nursing interventions, and client caregiver interventions. Care plans are organized by body systems to allow for quick retrieval of information. Both short-term and long-term outcomes are included in the care plans to aid evaluation of the care provided. Detailed assessment guidelines are provided for all body systems to facilitate complete and comprehensive client examinations. Guidelines for environmental and safety assessments aid in the appraisal and improvement of clients' living conditions. Client and caregiver interventions are outlined in the care plans to promote active client participation in self-care. The convenient pocket size makes transportation and use convenient to home health nurses. Appendices on documentation guidelines, laboratory values, medication administration, home care resources, and standard precautions provide quick access to useful home care information. Related OASIS items are identified in the assessment section, and ICD-9 diagnostic codes in the care plans section assist with proper home care documentation. Visit frequency and duration schedules are suggested within each care plan to assist nurses in evaluating and planning care. NANDA nursing diagnoses are consistent with the latest 2001-2002 nomenclature. An increase in suggested therapy referrals within the care plans and in a new appendix helps nurses identify indicators for specialized services. A fully updated Resources Appendix includes websites for easy access to home health service information.

Individuals with disabilities, chronic conditions, and functional impairments need a range of services and supports to keep living independently. However, there often is not a strong link between medical care provided in the home and the necessary social services and supports for independent living. Home health agencies and others are rising to the challenges of meeting the needs and demands of these populations to stay at home by exploring alternative models of care and payment approaches, the best use of their workforces, and technologies that can enhance independent living. All of these challenges and opportunities lead to the consideration of how home health care fits into the future health care system overall. On September 30 and October 1, 2014, the Institute of Medicine and the National Research Council convened a public workshop on the future of home health care. The workshop brought together a spectrum of public and private stakeholders and thought leaders to improve understanding of the current role of Medicare home health care in supporting aging in place and in helping high-risk, chronically ill, and disabled Americans receive health care in their communities. Through presentations and discussion, participants explored the evolving role of Medicare home health care in caring for Americans in the future, including how to integrate Medicare home health care into new models for the delivery of care and the future health care marketplace. The workshop also considered the key policy reforms and investments in workforces, technologies, and research needed to leverage the value of home health care to support older Americans, and research priorities that can help clarify the value of home health care. This summary captures important points raised by the individual speakers and workshop participants.

Unlike many specialties in the nursing profession, the home care nurse's work setting changes daily as they travel from location to location, home to home, visiting each assigned patient. Working alone most of the time, the home health nurse must be organized and autonomous. To further complicate things, many work for multiple agencies at the same time, and each agency has its own unique protocols for recording after-visit reports into their computer system at the end of the day. This handy Visit Tracker for Home Health Nurses is a one-of-a-kind system for gathering visit details into one bound location. Logically set up, it prompts the nurse to record all of the pertinent details about each visit as the day progresses, thus streamlining the process of inputting patient detail into each agency's computer program at the end of the day. Where appropriate, check boxes are provided to quickly indicate standard assessment elements, but blank write-in fields are also included for those areas where an entry is required. Each record includes a narrative section at the very end for capturing pertinent information not recorded elsewhere. A Patient Visit Log located at the front of the book where the Table of Contents typically is found can be updated daily for quick reference and includes: Patient Name, Visit Date, Agency Name, and Page Number. The journal will accommodate up to 60 visits and includes a sample completed assessment at the back of the book. · Gathers agency, patient, physician, medication, and medical history prior to any actual assessment or treatment. · Prompts the nurse through standard assessment items while also allowing for entry of additional non-standard notations. · Logical arrangement makes it quick and easy to extract all of the detail necessary for completing each patient file at the end of the day. · Chronological arrangement makes it easy to locate a specific record. · Patient Visit Log at the front of the book further enhances organization by letting the nurse set up their own Table of Contents. · Each patient record consists of a two-page spread, each page 8-1/2" x 11". · Each book will accommodate up to 60 patient visits. · Bound softcover format keeps everything in one place and fits easily into a supply bag.
