This handbook summarizes the data and techniques for hypofractionation and stereotactic radiation in a clinically-accessible way. Die Aufgabe dieser Arbeit war es den Ablauf der SBRT am Strahlentherapeutischen Institut in Klagenfurt zu untersuchen. Welche Behandlungserfolge konnten erzielt werden, welche geometrischen Unsicherheiten im Bodyframe von ELEKTA gilt es beim Planungszielvolumen zu berücksichtigen und welche Erwartungen gibt es für die Zukunft? CT- und EPI-Daten wurden verwendet um die systematischen und zufälligen Fehler zu eruieren. Um diese Lagerungsungenauigkeiten bei der Bestrahlung berücksichtigen zu können, wurde ein Sicherheitssaum für das Planungszielvolumen berechnet. Mit dieser Behandlungsmethode konnte eine lokale Tumorkontrollrate von 80% in primären Lungentumoren und 67% bei Metastasen erreicht werden, bei einer Jahres-Überlebensrate von 93.5% und einer 3-Jahres-Überlebensrate von 90%. Alle Patienten haben diese Behandlungsmethode gut vertragen. Die berechneten Sicherheitssäume gelten als Empfehlung für die Wahl des Planungszielvolumens am Strahlentherapeutischen Institut in Klagenfurt. *****Hypofractionated stereotactic body radiation therapy (hSBRT) is the treatment of choice in inoperable patients with primary non-small cell lung cancer and lung metastases. The aim of this thesis was to evaluate the treatment outcome and setup-accuracy achievable with the ELEKTA frame-based SBRT procedure of lung tumors at the radiotherapy department in Klagenfurt as well as to analyze the achievements made so far and to discuss perspectives for the future. Patient data of 41 patients were used to evaluate the accuracy of the treatment delivery process by investigating the magnitude of errors from different sources. CT-scans and electronic portal images were analyzed to determine systematic and random uncertainties. The calculated safety margins to account for the residual geometric uncertainties were 7.6 mm in cranio-caudal direction, 3.8 mm in lateral and 7.3 mm in anterior-posterior direction. A local tumor control rate of 80% was achieved in primary lung tumors and 67% in metastases. The one- to three-year overall survival rate was 93.5 % and 90 %, respectively. The hSBRT was well tolerated. The PTV-margins are considered to be recommendations, and further investigations should be made to examine detected errors.

This book is a comprehensive review of stereotactic radiosurgery (SRS) and stereotactic body radiation therapy (SBRT): its physics, clinical evidence, indications, and future directions. The utilization of stereotactic radiosurgery (SRS) and stereotactic body radiation therapy (SBRT) is increasing internationally because of several factors.
