ll 31 dicembre 2019, le autorità sanitarie cinesi notificano un focolaio di casi di polmonite ad eziologia non nota nella città di Wuhan (Provincia dell’Hubei - Cina). È in questo modo che nel grande paese asiatico, seminando il terrore, prende piede una nuova infezione virale che, nei casi più gravi, colpisce compromettendo in modo grave l’apparato respiratorio (polmonite interstiziale) con conseguenze fatali per moltissimi pazienti, in particolar modo anziani debilitati, con pregresse patologie o in età avanzata. La nuova ‘peste’ del XXI secolo, causata da un agente patogeno, il SARS-CoV-2 più conosciuto come Corona virus, porta il nome di "Sindrome respiratoria acuta grave coronavirus 2 (COVID-19)” e, come noto, si estenderà in tutto il vecchio continente prima e in tutto il mondo dopo provocando una “pandemia” che ad oggi, 21 giugno 2020, conta ben 8.708.008 casi confermati, 461.715 decessi e 205 paesi coinvolti. La pandemia porta con sé un lungo periodo di emergenza durante il quale bisogna fare i conti anche con il problema dello smaltimento dei cosiddetti rifiuti COVID-19 prodotti dalle strutture sanitarie, dalle aziende e dalle utenze domestiche. Un problema da valutare sotto un duplice aspetto: quello ambientale e quello del trasporto! La confusione è molta e gli operatori che a gran voce chiedono norme chiare e uniformi per tutto il territorio nazionale vedono invece arrivane una mole di decreti, circolari ministeriali, rapporti dalle autorità sanitare e le oramai famose ordinanze contingibili urgenti, strumento prima d’ora sconosciuto al grande pubblico. Queste norme, in nome dell’emergenza, fagocitano precedenti certezze del nostro ordinamento creando non poche perplessità e confusione tra gli operatori più sensibili. Ancora oggi, in piena fase 3, sperando che quanto passato rimanga solo come un triste ricordo, i problemi sono molti e non risolti come quelli relativi, ad esempio, alla carenza di impianti per lo smaltimento. Il presente lavoro nasce dalla volontà di dare a tutti gli operatori interessati, una sorta di vademecum da seguire per meglio orientarsi in quella giungla di norme, linee guida e ordinanze regionali che, sino ad oggi, non hanno certo brillato per chiarezza occupandosi di questi temi!

Oltre che trattare aspetti prettamente infettivologici, vengono infatti affrontate problematiche essenziali e di frequente riscontro nel paziente COVID-19, non solo nell’ambito internistico generale ma anche in altri ambiti specialistici. Gli argomenti affrontati potranno trovare utile applicazione sia nell’ambito ospedaliero che nell’ambito della medicina territoriale. Viene dato ampio spazio a indicazioni per un corretto supporto respiratorio, così come alla gestione di problematiche cliniche non prettamente inerenti alla patologia infettivologica ma frequenti in questi pazienti quali il controllo glicemico, la gestione dei disturbi idro-elettrolitici, il trattamento e il monitoraggio domiciliare. Viene altresì presentata una revisione critica dei dati di letteratura relativi al trattamento dell’infezione da SARS-CoV-2 (purtroppo ancora non consolidato e in divenire), nonché delle prospettive vaccinali.
